Introduction

This Provider Directory lists the current primary care providers and specialists available to you as a UCare member. You have access to primary care services through the primary care clinic you have chosen. You also have access to all of the specialists in this directory, including specialty clinics, chiropractors, vision care providers, and dental providers.

You must get all of your health care from UCare network providers. There are some exceptions. Examples of exceptions are: emergency or urgent care, open access services, and other previously authorized services. If you have questions about the providers you should use, please contact UCare Customer Services at 612-676-3200 or 1-800-203-7225 toll free, 8 a.m. to 5 p.m., Monday through Friday. At UCare, we make it easy to access the services you need when you need them. You can see all of the specialists in our network without a referral! Please refer to your Member Handbook for details about covered services and the list of services that require authorizations. Following is information on how to access several specialties.

UCare uses a network of mental health and chemical dependency providers in your community. You can get current information about those providers by using the Seach Network feature on or calling Customer Services at 612-676-3200 or 1-800-203-7225 toll free. Hours of service are 8 a.m. to 5 p.m., Monday through Friday. If you are hearing impaired, please call our TTY line at 612-676-6810 or 1-800-688-2534 toll free. After hours and on weekends, members can call the UCare 24/7 nurse line for assistance with finding provider information. The phone number for the UCare 24/7 nurse line can be found on the back of your Member Identification card.

UCare contracts with Delta Dental of Minnesota (Delta Dental) to manage your dental benefits. For more information on UCare's dental services, you can go online to , or call

the UCare Dental Connection at 651-768-1415 (local), or 1-855-648-1415 toll free from 7 a.m. to 7 p.m., Monday through Friday. TTY users can call 711. The UCare Dental Connection can help you find a Delta Dental CivicSmiles? provider or dental home; schedule dental appointments, including appointments for follow-up and specialty care; coordinate transportation to dental appointments; coordinate interpreter services for dental appointments; and answer dental benefit and claims questions.

In an emergency that needs treatment right away, either call 911 or go to the nearest emergency room. In other cases, call your primary care doctor's office for instructions on how to get urgent care or after-hours care. The number is answered 24 hours a day. You can also call the UCare 24/7 nurse line at the number on the back of your Member ID card. The UCare 24/7 nurse line is answered by nurses 24 hours a day, seven days a week. You must call UCare's Customer Services Department at 612-676-3200 or 1-800-203-7225 toll free within 48 hours, or as soon as you are able, if you are admitted to an outof-network hospital.
