I hereby consent to and authorize the owner’s/landlord’s tenants screening service company or anyone working on behalf of the owner/landlord to obtain, verify and exchange information on any reports concerning me/us as maintained by, but not limited to: City, County, State and Federal Law Enforcements Agencies, credit reporting agencies, past and/or present employers, creditors, friends and current or past landlords I understand that any information obtained may be considered by the landlord in their discretion, as a factor in decisions they make, with respect to the apartment for which I/We are applying.

I hereby release and hold harmless: agents, owners and affiliates of, but not limited to: their officers, directors, employees, law enforcement agencies, credit reporting agencies, present or past employers and present or past landlords, who provide information.

I hereby certify that the information contained in this rental application is accurate, full and complete. Any discrepancy or lack of information will result in immediate rejection of this application. I/We understand that this is an application for an apartment and does not constitute a rental agreement in whole or in part. A deposit of $ is being held towards the first month’s rent and is non-refundable if you are accepted and fail to move in. I/We hereby acknowledge a non-refundable fee of $ to be used to check the credit worthiness of the applicants.

Base rent and other monthly charges are due and payable on the first day of each month in advance.

Pursuant to Massachusetts Law, the Management shall not make any inquiry concerning the race, religious creed, color, national origin, sex, sexual orientation, age (except if a minor), ancestry, or marital status of the applicant, or concerning the fact that the applicant is a veteran or a member of the armed forces or handicapped. The applicant authorizes the Management and/or Renting Agency to obtain or cause to be prepared a consumer credit report relating to the applicant.

Neither the Owner nor the Management is responsible for the loss of personal belongings caused by fire, theft, smoke, and water or otherwise unless caused by their negligence.

The undersigned warrants and represents that all statements herein are true and agreed to execute upon presentation a Lease or Tenant at Will agreement in the usual form, a copy of which the applicant has received or has had occasion to examine, which lease or agreement may be terminated by the Lessor if any statement herein made is not true. Deposit is to be applied pursuant to Massachusetts law, or applied to actual damages sustained by the owner except it is to be refunded if this application is not accepted by the owner. This application and deposit are taken subject to previous applications.

I/We understand that this is an application for mobile home rental and does not constitute a rental agreement in whole or part. Any person or firm is authorized to release information about the undersigned upon presentation of this form or a photocopy of this form at any time.
