AVID Tutorial Make-up Worksheet

AVID Tutorial Make-up Worksheet

I was unable to attend the AVID Tutorial on __________________ because:

I plan to do the following in order to make-up the tutorial session I missed because of my absence:

Please check one activity from the list of suggestions below.*

□ Attend at least one 30 minute session of tutoring for one of your academic classes (outside of class time). Include the class name, topic, your Cornell notes, and a summary of what you learned. Your attendance must be verified by the teacher present at tutoring (teacher signature required with times).

□ Read an article (found in a magazine, newspaper, or on the Internet) about a college-related topic, a career that interests you, or advice on getting a job. Take one full page of Cornell Notes which may include a summary of the article. Submit your notes with a copy of the article. (Make sure that you choose an appropriate article or you will not receive full credit - getting prior approval would be a good idea.)

□ Design a poster about AVID to be displayed in the classroom or office. What is AVID? What does it mean to be an AVID student? How will AVID help you on your journey to college? Must be colorful, creative, and very neat. Your finished poster must reflect good effort and appropriate time spent on the task.

□ Tutor another student in any curriculum for at least 30 minutes outside of class time. Complete the AVID tutorial worksheet. Include the student’s name, state a problem and the curriculum area, summarize your procedure for helping the student, and the amount of time you spent. A supervising adult (parent/guardian or teacher) must sign the tutorial worksheet to verify the amount of time you spent on task.

□ Interview a college student (someone currently enrolled). You must interview the student by e-mail, asking a minimum of 10 questions. Thank the student for answering your questions after he/she responds with answers. Print out and turn in the entire correspondence between the two of you.

□ Other_______________________________ (You must get prior approval from your AVID teacher for any alternate assignment you propose.)

*Make-up Work Reminders:

• Make sure that you have appropriate signatures (when necessary) and materials to turn in! If you do not have signatures you will not receive full credit.

• You must turn in this form with your make-up work.

• All make-up work should be turned in within 2 weeks of absence.
