District Migrant Coordinators:

The Florida Exemplary Migrant Education Awards Project celebrates the successes of Florida’s graduating migrant high school seniors, migrant parents, school support personnel, and community partners.

You are encouraged to submit names of persons to be nominated for the various categories during the state selection process. Attached are the applications for the 2011 Florida Exemplary Migrant Education Awards.

In an effort to increase the number of participants, please share the attached forms with staff, community members, and other stakeholders. They may wish to submit nominations for your review, approval, and signature. Please submit applications electronically by March 15, 2011.

A review committee will process the applications and select winners. The 2011 Florida Exemplary Migrant Education Book, a publication highlighting award winners, and their biographies will be made available. Traditionally, all state winners are recognized at an Awards Luncheon during the Florida Migrant Education Conference.

Each district is allowed to submit only one male nominee and one female nominee in each category, with the exception of the Migrant Scholars applications. Districts may submit as many Migrant Scholars applications as deemed eligible. All students not selected for the Most Exemplary Migrant Scholar of the Year Award will be considered for the Migrant Scholars recognition. Additionally, all candidates for graduation will be listed in the book.

Please Note:

Only electronic (not scanned) 2” x 3” or 4” x 5” photographs and one page electronic- single spaced biographical profiles in Times New Roman 12 pt font will be accepted.

Make sure that the biographies of all nominees have been written by persons other than the nominee, and edited by you, or your supervisor, if in instances where someone submits you as a nominee for an award category. It is important that this one-page document be “print ready” (but not scanned).

Districts are requested to provide (for publication) an electronically submitted alphabetical list(not faxed or scanned) of all migrant students (first and last names), who have been identified as migrant at least one day during their high school enrollment and are candidates for graduation in the spring or summer of 2011. Please also indicate the high school they attend.

Realizing the uncertainty of the graduation status of some of the identified students, all will be recognized as Candidates for Graduation.

2011 Most Exemplary Migrant Scholar

Each District may nominate only one male and one female student for this honor.

Applications must include the following:

1. Parent Guardian Permission/Release Form (attached)

2. An electronic photograph (not scanned): (2” x 3” or 4” x 5”);

3. A COE (Certificate of Eligibility) certifying that the nominee was a migrant student (at least one day) during high school (grades 9-12);

4. Verification of passing both sections of the FCAT;

5. Transcript highlighting the student’s un-weighted state GPA; and

6. A one page biographical profile for publication must be written by the Migrant Staff or School Personnel (not the nominee) and proof read by the Migrant Program Coordinator prior to submission for each nomination. The district approved profile and photos MUST be submitted electronically in a word document.

The following information must be in the profile:

a. Details of migrant background;

b. Examples of good citizenship;

c. Awards, honors, or special achievements;

d. Hardship and/or challenges encountered while pursuing an education;

e. Future plans and personal goals of student; and

f. Reference to the student’s state unweighted GPA must appear in the body of the biographical profile.

7. Documentation from the High School Guidance Department that the nominee will meet at least two of the following four criteria:

a. Is currently in the top 15% of his/her graduating class;

b. Has at the time of nomination, a minimum overall 3.0 un-weighted State GPA;

c. Successfully completed (or is currently enrolled in and is passing) seven (7) full credits in Honors, Dual Enrollment, and/or Advance Placement Classes. Please provide a transcript highlighting these courses in yellow highlighter. If the student is currently enrolled in these courses, then a letter from the teacher or guidance counselor stating that the student is currently passing the classes must be included; or

d. Was formally invited to participate as a member of the National Honor Society while in high school (Some migrant students have been eligible to participate, were invited, but due to family constraints were unable to join. These students qualify under this definition).

STATE GPA CALCULATIONS APPEAR ON ALL OFFICIAL TRANSCRIPTS. In fairness to all applicants, please report the state (unweighted) GPA when compiling the biographical profile/story. Also highlight the overall state unweighted GPA when submitting supporting documentation.

2011 Rising Migrant Scholar

Each District may nominate only one male and one female student for this honor.

This would be someone who has overcome great obstacles and/or has shown positive improvements on academics.

Application must include the following:

1. Parent Guardian Permission/Release Form (Attached);

2. An electronic photograph: (2” x 3” or 4” x 5”)-not scanned;

3. A COE (Certificate of Eligibility) certifying that the nominee was a migrant student (at least one day) during high school (grades 9-12);

4. Documentation from the High School Guidance Department if student is being nominated for academic improvement;

5. Documentation outlining the extent of improvement; and

6. A one page biographical profile.

The following information must be in the profile:

a. Details of migrant background;

b. Examples of good citizenship;

c. Awards, honors, or special achievements;

d. Hardship and/or challenges encountered while pursuing an education biographical profile for publication must be written by the Migrant Staff or School Personnel (not the nominee) and proof read by the Migrant Program Coordinator prior to submission;

e. Description of improvement; and

f. Future plans and personal goals of student.

2011 Migrant Scholars

All eligible students may be nominated by each district if they meet the following criteria.

1. Application must include the following: Parent Guardian Permission/Release Form (Attached)

2. An electronic photograph: (2” x 3” or 4” x 5”);

3. A COE (Certificate of Eligibility) certifying that the nominee was a migrant student (at least one day) during high school (grades 9-12);

4. Verification of passing both sections of the FCAT.

5. A one page biographical profile for publication must be written by the Migrant Staff or School Personnel (not the nominee) and proof read by the Migrant Program Coordinator prior to submission for each nomination.

The following information must be in the profile:

a. Details of migrant background;

b. Examples of good citizenship;

c. Awards, honors, or special achievements;

d. Hardship and/or challenges encountered while pursuing an education; and

e. Future plans and personal goals of student.

6. Documentation from the High School Guidance Department that the nominee will meet at least two of the following four criteria:

a. Is currently in the top 15% of his/her graduating class;

b. Has at the time of nomination or graduation, a minimum overall 3.0 state (un-weighted) GPA;

2. Three letters of support (at least one from the direct Supervisor and/or District Supervisor); and

3. A one page biographical profile for publication must be written by the Migrant Staff or School Personnel (not the nominee) and proof read by the Migrant Program Coordinator prior to submission for each nomination. The district approved profile and photos MUST be submitted electronically.

The following information must be in the profile:

a. Job performance above and beyond the call of duty for migrant children and families;

b. Details of migrant background (if applicable) or Educational background;

c. Special achievements and awards especially to work w/migrant students;

d. Hardship and/or challenges encountered (if applicable)

e. Details of community service;

f. Special assistance provided to the Migrant Education Program/students (if applicable); and

g. Career highlights with migrant education.

2011 Migrant Advocate

The nominee should be an Advocate at any school, district, regional, or state level, who has contributed to the improvement of the quality of life for migrant students, their families, and peers.

Application must include the following:

1. An electronic photograph: 2” x 3” or 4” x 5”;

2. Three letters of support (at least one from the direct Supervisor and/or District Supervisor); and

3. A one page biographical profile for publication must be written by the

Migrant Staff or School Personnel (not the nominee) and proof read by the Migrant Program Coordinator prior to submission for each nomination. The district approved profile and photos MUST be submitted electronically in a word document.

The following information must be in the profile:

2. Three letters of support (at least one from the District Supervisor); and

3. The nominee(s) for Migrant Health Professional can be an entity or an individual who has provided health services to migrant children and their families.

Application must include the following:

1. An electronic photograph of the group or the individual : 2” x 3” or 4”x x 5”;

2. Three letters of support (at least one from the District Supervisor); and

3. A one page biographical profile for publication must be written by the Migrant Staff or School Personnel (not the nominee) and proof read by the Migrant Program Coordinator prior to submission for each nomination. The district approved profile and photos MUST be submitted electronically in a word document.

The following information must be in the profile:

a. Exemplary service to migrant community;

b. Details of migrant background (if applicable);

c. Role modeling or leadership guidance to child or community;

d. Level of involvement with students or school; and

e. Hardship and/or challenges encountered.

2011 Migrant Home-School Liaison

Application must include the following:

1. An electronic photograph: 2” x 3” or 4” x 5”;

2. Three letters of support (at least one from the direct Supervisor and/or District Supervisor); and

3. A one page biographical profile for publication must be written by the Migrant Staff or School Personnel (not the nominee) and proof read by the Migrant Program Coordinator prior to submission for each nomination. A one page biographical profile for publication must be written by the Migrant Staff or School Personnel (not the nominee) and proof read by the Migrant Program Coordinator prior to submission for each nomination. b. Role/modeling or leadership to their child or community;

2011 Migrant Recruiter

Application must include the following:

2. Three letters of support (at least one from his/her direct Supervisor and/or District Supervisor); and

The nominee for Migrant Success Story must be an adult (migrant, former migrant, or the child of migrants) who has achieved prominence in his/her chosen field of work.

Application must include the following:

1. An electronic photograph: 2” x 3” or 4” x 5”;

2. Three letters of support (at least one from the District Supervisor); and

3. A one page biographical profile for publication must be written by the Migrant Staff or School Personnel (not the nominee) and proof read by the Migrant Program Coordinator prior to submission for each nomination. The district approved profile and photos must be submitted electronically in a word document.

The following information should be included in the profile:

a. Details of migrant background;

b. Role/modeling or leadership to their child or community;

c. Level of involvement with child or school;

d. Hardship and/or challenges encountered and overcome; and

e. Family story.

2011 Migrant Support Services Person

The nominee for Migrant Support Services must be an adult who has achieved prominence in his/her area of service to migrant families through office support. This category can cover a variety of clerical and miscellaneous areas that may be specific to a region.

Application must include the following:

1. An electronic photograph: 2” x 3” or 4” x 5”;

2. Three letters of support (at least one from the District Supervisor); and

3. A one page biographical profile for publication must be written by the Migrant Staff or School Personnel (not the nominee) and proof read by the Migrant Program Coordinator prior to submission for each nomination. The district approved profile and photos MUST be submitted electronically in a word document.

The following information should be included in the profile:

a. Details of migrant background (if applicable);

b. Special achievements and awards;

c. Special assistance provided to the Migrant Education Program (if applicable);

d. Hardship and/or challenges encountered (if applicable);

e. Job performance above and beyond the call of duty for migrant children and families;

Migrant Teacher/Educator

The nominee should be a Teacher/Guidance Counselor/Teacher on Assignment/Non Administrator/etc. at any school, district, regional, or state level, who has contributed to the improvement of the quality of life for migrant students, their families, and peers.

Application must include the following:

1. An electronic photograph: 2” x 3” or 4” x 5”;

2. Three letters of support (at least one from the direct Supervisor; and

3. A one page biographical profile for publication must be written by the Migrant Staff or School Personnel (not the nominee) and proof read by the Migrant Program Coordinator prior to submission for each nomination. The district approved profile and photos MUST be submitted electronically in a word document.

The following information should be included in the profile

a. Details of migrant background (if applicable);

b. Special achievements and awards;

c. Special assistance provided to the Migrant Education Program;

2011 Agribusiness Partner of the Year

The nominee should represent an agribusiness that has demonstrated support of the Migrant Education Program, students and/or families at the school, district, regional, or state level.

Application must include the following:

1. An electronic photograph: 2” x 3” or 4” x 5”;

2. Letter of support from local MEP supervisor; and

3. A one page biographical profile for publication must be written by the Migrant Staff (not the nominee) and proof read by the Migrant Program Coordinator prior to submitting. The district approved profile and photos MUST be submitted electronically in a word document.

Samplings of the following information must be in the profile:

a. Details of exemplary service/access to migrant child, migrant family and/or migrant community;

b. State level of involvement with migrant students, families and/or staff; and

c. Explain how the nominee has contributed to improving the quality of life for migrant students and families.

REQUIRED INFORMATION AND SUPPORTING DOCUMENTS

For Most Exemplary Migrant Scholar, Rising Migrant Scholar, and Migrant Scholar nominations, a copy of the latest Migrant Certificate of Eligibility (COE) and a high school transcript are required (do not send electronically). Nominee must have been a migrant student (at least one day) during high school (grade 9-12) enrollment. Hard Copies of transcript and COE must arrive in the mail. In the packet please include a cover letter listing all nominees from your district. A hard copy of each completed biography (with photo) is also required. Please mail to:

Migrant Certificates of Eligibility and Transcripts are not required for the rest of the students who are on the general list of Candidates for Graduation.

For all nominees, an electronic one-page biographical profile (which will be published) written by a local or District Educator (not the nominee) and Proof Read by the Migrant Coordinator must be submitted. The biography will also serve as a letter of reference for the nominee. The profile must be limited to one (1) page, and be written in Times New Romans 12pt font (single spaced). Please do not compress files. Send each biographical sketch as a separate attachment.

All nomination packages must include an electronic (not scanned) wallet-sized photo of the nominee.
