Before Writing the Evaluation

Self-Assessment

Prior to completing the evaluation for the performance period, provide the employee with the SelfAssessment form.

Tip: Provide a copy of the completed performance

evaluation to the employee at the end of

the day before the scheduled performance

evaluation meeting.
