LIFE INSURANCE
• 24-hour coverage against loss of life due to natural or accidental causes
• Insurance protection without geographic limits

CLASSIC CORPORATE PERSONAL ACCIDENT
Accidental Death – Payment of the Principal Sum when loss of life results from an
accident or injuries caused by an accident.
Accidental Dismemberment and Loss of Use – Provides a percentage of the
Principal Sum corresponding to a schedule when injury results in an Accidental
Dismemberment or Loss of Use of various parts of the employee’s body.

Accidental Permanent Total Disability – A monthly cash benefit equal to 3% of the
Principal Sum paid on the 7th month of continuous, permanent and total disability for
32 months, and 4% of the Principal Sum on the 33rd month.
Coverage Limitation for Murder – Pays up to 100% of the Principal Sum, but not exceeding P 2M for injuries due to murder or any attempt thereof. Up to 100% of the benefits payable for injuries due to homicide or any attempt not occasioned by provocation of the Member.
Flying Coverage - Pays a benefit if the employee suffers an injury while a passenger of
a commercial aircraft.
Renewal Bonus – Offers an increase of five percent (5%) on the original Principal
Sum upon renewal of the Policy, each year up to a maximum of five (5) consecutive
years.

General Coverage Guidelines
1. Rates are inclusive of the government-mandated Documentary Stamp Tax equivalent to P0.50 per P200.00 of total premium.
2. Eligibility: All regular, full-time, actively at work employees, ages 18 – 64.
3. Participation Requirement: If the plan is on a contributory basis, at least 75% of all eligible employees are required to enroll. If the plan is on a non-contributory basis, a 100% participation is required.
4. Suicide is not covered on the first year.
Classic Corporate PA Guidelines
□ Eligibility: All regular, Active member in good standing, actively performing the daily normal chores of life, ages 18 – 64.
□ Maximum coverage is 10x the annual income or P20 million, whichever is less
□ The maximum aggregate liability of the Company per any one accident under the Policy shall be 200 times the annual premium being paid on the policy, or P 2,500,000, whichever is lesser. In the event the said maximum aggregate liability for any one accident is insufficient to pay the full amount of indemnity for each and all insured persons, then the amount payable to each Insured shall be reduced in proportion to the maximum aggregate liability for any one accident. In the event of a single accident involving several insured persons, any amount of premiums paid in excess of the premiums corresponding to the maximum aggregate liability for any one accident will be refunded to the Assured
□ EXCLUSIONS - Any loss or expense caused by or resulting from the following will not be paid:
1. Suicide
2. War, Invasion or Act of Foreign Enemy
3. Service in the Armed Forces of any country or international authority whether in peace or war.
Philamlife reserves the right to withdraw or modify the proposal should there be any material change in the underwriting assumptions such as the nature of business, number of lives, etc.
