Request for Proposal for MHA HEN 2.0 Quality and Patient Safety Mini-Grants Proposal Deadline: Jan. 29, 2016 OverviewThe Minnesota Hospital Associations’ (MHA) Partnership for Patients (PfP) Hospital Engagement Network (HEN) is looking to engage organizations to strengthen and expand their work in quality and patient safety.To assist members, MHA is offering grant awards with funding up to $10,000 for hospitals to work on 6 month projects within their organization. Grant awards can support specific new initiatives in HEN 2.0 topic areas or quality and safety work already in progress at an organization. Examples of previous mini-grant awards are included in appendix A. Proposal content will be scored as follows:Describe the quality/patient safety topic that your team will address, providing a high level overview of the proposed project and why you feel this is important. (10 points)Project plan: Tell us about your project. Identify the tasks, milestones and supporting activities you plan to accomplish. (15 points)Measurement: Describe how you plan to measure the progress towards your project goals. (10 points)Inclusion and education of staff/providers, patients and families: Describe your plans for including and educating staff/providers, patients and families in this project. (15 points)Provide a budget that aligns spending with interventions being implemented. A brief narrative justification of expense should be included for each budget category. *Please note that budgets may not include food expenses per government contract regulations.Applicants are encouraged to review topic-specific roadmaps and gap analyses on the MHA website for additional guidance and resources.Project ExpectationsProject period begins Feb. 8, 2016 and ends Aug. 8, 2016.Awardees will be expected to:Prepare brief monthly progress reports.Participate in round robin learning calls with other hospitals working on this topic.Prepare and submit a final project report outlining project activities and outcomes by Sept. 2, 2016. Share project results during future webinars/presentations.Eligibility CriteriaOnly hospitals currently participating in the MHA HEN are eligible to applySelection CriteriaProposals will be reviewed based on demonstrated ability to meet the above criteria and expectations, level of innovation and anticipated impact of the project. Selected grant recipients will be notified by Feb. 5, 2016.Funding will be provided to the top scoring proposals. Total number of projects funded will depend on amount of funding requested.
