FORM 1099-HC (REQUIRED FOR MASS. STATE TAX RETURN)

Form 1099-HC is needed to complete your MA state tax return. This form serves as proof of health insurance coverage for Massachusetts residents age 18 and older. Your 2019 Form 1099-HC will be mailed to you by your health insurance carrier by January 31st, 2020.

Note: This proof of coverage is REQUIRED for your Massachusetts state income tax filing. The Health Benefits & Insurance Department does not have access to print this form on behalf of employees.
