Rationale for listing Glucometer procedure as training versus delegation:
A delegation ideally includes only a few chosen individuals of a larger group, performing a task frequently enough to maintain a level of competency, e.g. gastric tube feeding. Training such as CPR training, typically involves training everyone in the group, in preparation of an anticipated emergency situation. In our setting, the glucometer is to be used by unlicensed staff in situations where there is no licensed LPN’s or RN’s stationed, in low frequency occurrences and emergency situations. All individuals of the group will be trained yearly. Those who fail to demonstrate initial competency will be retrained. The step-by-step procedure will be posted and readily available for all staff trained to use the glucometer. This rationale provides higher quality of care and eliminates the excess blood collections needed for delegating the glucometer task from the individual client. In order to provide 24 hour quality care all staff need to be trained to perform the needed blood glucose monitoring of the client.
Blood Glucose Monitoring:
Blood glucose monitoring is essential in taking care of a client you know, who has diabetes. Checking a CBG (capillary blood glucose), will help you learn how food activity levels, stress, medicine and insulin change blood sugar levels. Knowing the blood sugar level promotes health and prevents or delays complications associated with diabetes such as blindness and kidney failure.

Procedure:
Checking blood sugar: A clinician / Primary Care Physician (PCP) order must be obtained indicating how often to check blood sugar. There need to be parameters indicating when to notify a Health Care Professional.

Preparations:
1) Verify “clinician” orders.
2) Wash you hands and put on gloves
3) Approach the individual whom you will be testing
4) Explain what you will be doing and ask for their help.
Procedure:
1) Load the 10-test disc as necessary.
2) Prepare lancet device.
3) Wipe individual’s finger with alcohol wipe where blood sample will be obtained. Allow wiped area to dry before puncturing the skin.
4) Place lancing device to area that has been cleaned with alcohol.
5) Trigger lancing device to release Lancet into the skin to cause a small puncture.
6) As a small blood drop appears remove lancing device and pick-up meter containing test strip.
7) Hold tip of test strip next to the droplet of blood and allow test strip to draw up the droplet of blood. Blood is pulled IN the test strip, not dropped ON. Do NOT add more blood to the test strip after the meter beeps.
8) When the right amount of blood has been obtained in the test strip the meter will beep and start counting down numbers. When complete the Blood Glucose meter will reveal the Blood sugar value.
9) Log Blood sugar value in the Client’s MARS. (DHS 4574 Medication Administration Record)
Final process:
1) Have client hold pressure to puncture site with a cotton ball. Wipe any excess blood from individual’s finger and cover with a Band-Aid.
2) With hands gloved release test strip from meter.
3) Place used lancet in a sharps container.
4) Dispose of any contaminated items such as alcohol wipes, cotton balls in an appropriate reciprocal.
5) Clean meter and Lancet device and place in Diabetic kit.
6) Remove gloves and wash hands.
