Parent Permission Form

My child, _________________________________________, who attends Grady High School, has permission to travel to and attend the Houston County HS Speech and Debate Tournament in Warner Robins, GA. I am hereby giving my express consent for Mario Herrera, teacher at Grady High School, to travel by school bus to and from the tournament. I am aware that my child is not required to accept the transportation offered and that transportation for my child to accept the foregoing event is being made available as a courtesy in order to ensure that my child has the opportunity to participate in the event. I further understand and agree, for myself and my child, that neither Mario Herrera, nor the Atlanta Board of Education, the Atlanta Public School system, its officers, agents or employees shall have any liability for any injury or damage to my child’s person or belongings arising from transportation to or attendance of the tournament specified in this waiver.

My child has permission to attend the Houston County HS Speech and Debate Tournament in Warner Robins, GA. I understand that although every effort will be made to provide for the safety of my child, Henry W. Grady High School and/or the accompanying sponsor and/or driver cannot be personally held responsible or liable for any aspect of this trip. My signature absolves the Atlanta Board of Education, the Atlanta Public School System, Grady High School, and Mario Herrera from all legal and financial responsibility in connection with this trip.

I further give Mario Herrera permission to seek medical assistance for my child should my child need such attention. My signature serves as a medical release in such a situation.

TOURNAMENT INFORMATION

We will be leaving from Grady HS at 6:30am on Saturday, October 30, 2010. A school bus will pick us up in front of the cafeteria on Eighth Street. This same area will serve for the drop-off point upon our return, hopefully no later than 10:00pm. We should arrive at Houston County HS by 8:00am. The competition will take place at the high school for the entirety of the day. Students need to be in competition dress (business attire) and either bring snacks/meals for the day, or money for such. A snack bar will have food for students.
