Medical Marijuana Registry Application Steps

Before you apply

Check your eligibility

To apply you must

? Be 18 or older ? Be a Colorado

resident ? Have a qualifying

medical condition

See your Physician

Physician must be

? An MD or DO ? Licensed in Colorado ? Registered to submit

physician certifications online

Gather required documentation

Find a full list of required documents at cdphe/ medicalmarijuana

Create an account

1. Visit medicalmarijuana.

2. Login using your username and temporary password

3. Change your password the first time you login

Tip: Choose a username and password that is easy to remember

Apply

1. Login

2. Click "new"

? New patients and renewal patients using the system for the first time should click "new".

3. Enter patient information

? If you plan to buy your medical marijuana from a center, choose "yes" for the question "Once you receive your registry card, will you get all of your medical marijuana from a center (dispensary)?"

? Provide your current, valid Colorado ID or driver's license information. ? Double-check everything before you click "save and submit." ? You won't be able to change anything until after your registration is approved.

4. Add physician certification

? Click the "Physician certification" tab ? Click "new" ? Choose your physician certification from the drop down menu ? Click "save"

5. Enter cultivation information

? Skip this step if you answered "yes" to the question "Once you receive your registry card, will you get all of your medical marijuana from a center (dispensary)?"

6.Pay

? Use a credit card, debit card or electronic check.

After you apply

Check your email

You'll receive a notification from the registry telling you to login to your account and check your status

Print or display your card on a mobile device

Apple users will need to download the Adobe Acrobat Reader app

If you applied by mail and need access to your digital card, complete a request for

online access.
