SNAP Outreach Pilot Program – Dual Eligibles
Purpose- description of function of the program and reason why it was created:
The purpose of this RFA is to secure services from a not-for profit firm via the North Carolina Division of Social Services Supplemental Nutrition Assistance Outreach Pilot Program to provide outreach efforts to seniors aged 65 and older who are dually enrolled in Medicare and Medicaid who qualify for the Supplemental Nutrition and Assistance Program (SNAP) or Food and Nutritional Services (FNS) but are not currently enrolled.
Description- Brief statement of nature and scope of the program:
The SNAP Outreach Pilot Program is to increase access to public benefits, improve health, promote independence, and reduce health care cost for seniors aged 65 and older who are dually enrolled in Medicare and Medicaid but are not currently enrolled in SNAP/FNS.
SNAP Outreach raises awareness of the nutrition benefits of SNAP/FNS, eligibility rules, and how to apply. SNAP outreach corrects myths and misperceptions about SNAP/FNS and enables potentially eligible people to make an informed decision to participate
Eligibility:
Any non-profit firm that has successfully demonstrated experience in assisting with these specified types of outreach services and the partnership shall accomplish each of the following:
1) Identify dual eligible seniors age 65 and older who qualify for SNAP/FNS but are not enrolled
2) Conduct an outreach program towards those seniors for the purpose of enrolling them into SNAP/FNS.
3) Provide comprehensive application assistance through outreach specialists to complete SNAP/FNS applications.
4) Make recommendations regarding policy options available to the State to streamline access to benefits
5) Evaluate project effectiveness and explore how data can be utilized to achieve optimal outcomes

▪ Applicants and subcontractors must follow E-Verify requirements found in HB786 (). This requires contractors and the contractor’s subcontractors to comply with the requirements of Article 2 of Chapter 64 of the General Statutes ().
▪ Applicants’ Internal Revenue Service (IRS) exemption letter must be current within five years.
▪ Applicants’ Employer Identification Number (EIN), IRS tax exemption status documents, and registration with the Secretary of State must be consistent with both the name of agency and the EIN provided.

How to Apply:
The Request for Application (RFA) may be sent via email with required forms to interested agencies and organizations beginning January 3, 2017.
Applicants will submit an email application. Faxed applications will not be accepted.
Any agency or organization that plans to submit an application should send a letter regarding NOTICE OF INTENT prior to submitting the application. The Letter of Intent may either be mailed or sent as an e-mail attachment to connie.dixon@dhhs. no later than 5:00 p.m. on January 20, 2017.

Nothing may be added to any application after it has been submitted. Eligible applications will then be forwarded to a grant review committee who will review, score and rank the applications
