1 REFERRAL PROCESS
THE REFERRAL PROCESS FOR MONARCH IS SIMPLE:
Monarch Intake Coordinator receives a call from a Physician, Case Manager, Discharge Planner, patient or patient’s caregiver.
The Intake Coordinator helps to define the services needed and records information pertinent to patient’s health care needs.
The Intake Coordinator initiates and coordinates the Assessment and evaluation with the Admitting Nurse, referral source and the patient. The Admitting Nurse makes a home visit to determine the patient’s needs and admit the patient for care within 48 hours.
A patient care team leader receives the assessment and coordinates all disciplines, equipment and supplies needed to care for the patient in his or her home as well as determining the initial frequency of visits and directs staff to begin giving service.
Services are provided 365 days a year.
Monarch observes a standardized pricing for all services and will gladly prepare an itemized statement to a patient or third party payer.

Our nurses, therapists, home health aides and technicians are experienced, credentialed professionals. We work as a team using Physicians Orders and sound techniques as a basis for a Plan of Care, thus enhancing recovery in the private and comfortable environment of the patient’s home.
