This directory is for Benton, Clackamas, Douglas, Jackson, Josephine, Lane, Linn, Marion, Multnomah, Polk, Washington, and Yamhill counties in Oregon and Clark County in Washington.

This directory is current as of September 2015. Some network providers may have been added or removed from our network after this directory was printed. We do not guarantee that each provider is still accepting new members. To get the most up-to-date information about SilverSneakers contracted facilities in your area, you can visit or call SilverSneakers Customer Service at 1-888-423-4632, Monday through Friday, 5 a.m. to 5 p.m. PST, except holidays. Health Net has a contract with Medicare to offer HMO, PPO, HMO SNP coordinated care plans. Enrollment in a Health Net Medicare Advantage plan depends on contract renewal. This information is available for free in other languages. Please call our Member Services number at 1-888-445-8913. From October 1 through February 14, our office hours are 8:00 a.m. to 8:00 p.m., 7 days a week, excluding certain holidays. Section 1 ? Introduction

This directory provides a list of participating SilverSneakers contracted fitness clubs and exercise centers. SilverSneakers benefits are available to members of Health Net Ruby, Jade, Aqua, Violet Option 1, and some Violet Option 2 plans. To get detailed information about your fitness and exercise coverage, please see your Evidence of Coverage (EOC). SilverSneakers? has a contract with Health Net.

Membership Choices

The SilverSneakers program allows members to choose one of two membership options: 1. SilverSneakers Facility Membership is a membership at a participating fitness club or exercise center. ? Members may choose from the list of participating SilverSneakers facilities to receive a no-cost standard

membership. The membership agreement that you may be required to sign at the fitness facility is for a "standard fitness facility membership at no additional cost," which includes the covered services available through the program. 2. SilverSneakers Home Exercise Program is a home-based fitness and health education program for

members who do not have access to a participating fitness facilities or prefer to work out at home.

To Enroll

1. Simply choose a fitness facility by using this directory, going online to , or by calling SilverSneakers Customer Service 1-888-423-4632 (TTY users should call 711), Monday through Friday, 5 a.m. to 5 p.m. PST, except holidays.

2. Visit a participating fitness facility to sign a membership agreement. 3. If you choose to access fitness facility services otherwise available by the facility at an additional fee, then

the agreement you have with the fitness facility may reflect costs associated with those non-program related services. 4. If you choose the SilverSneakers Home Exercise Program, you can enroll online at , or by calling SilverSneakers Customer Service 1-888-423-4632 (TTY users call 711), Monday through Friday, 5 a.m. to 5 p.m. PST, except holidays.
