2. SECTION B ("School Choice Information"): From the list of participating schools below, write the name of the charter school you wish to apply to. a. Do not submit this application to any school not included in the list of participating schools. b. STUDENTS APPLYING TO MORE THAN ONE CHARTER SCHOOL: You must fill out a separate application for each charter school you are applying to. Do not write in more than one school in Section B ("School Choice Information").

3. SECTION C ("Sibling Information"): Complete Section C if the student has a sibling currently at the charter school that you are applying to. Many, but not all, charter schools give lottery preference to applicants with sibling(s) enrolled in the school. You can review all lottery preferences by checking the school's profile at .
