This directory lists dental providers you may use with Blue Cross Community Health Plans. These dental providers are part of the "Plan" and we call this group "network providers." They have signed a contract with us to give you services. The Directory list is up-to-date as of the date on the front and back cover, but you need to know that:

? Some network providers may have been added or removed from our network after this directory was printed.

? Some of those listed may not be taking on new members. If you need help finding a Provider who will take new members, call Member Services at the number below.

Getting Started

Blue Cross Community Health Plans has partnered with DentaQuest to provide dental services. You do not need a prior authorization to visit a dentist. You can use this directory to find a dental provider. You can also visit DentaQuest's website at to find an in-network dental provider. To talk to a live agent to find a dental provider, call Member Services at 1-877-860-2837, TTY/TDD: 711.

This section explains key terms:

Providers are dentists who offer care and services. Services include annual checkups, cleanings and other treatments for your teeth. Network providers serve members in our Plan. When you see a network provider, you usually pay nothing for covered services.

Choosing a Dentist

1. Choose a dentist who is in our network and accepting new patients. You can go to the list starting on page 1 and choose a dentist: ? That you use now; or ? Who has been suggested by someone you trust; or ? Whose offices are close to you.

2. Schedule an appointment by calling the doctor or the medical group at the phone number listed! The provider will request your insurance information, so have your Member ID Card ready.

The types of dentists listed in this directory are: ? General Dentist. This is your primary care dental provider. They diagnose, treat and manage your overall dental needs. Common services offered by general dentists include routine cleanings, fillings, and preventative education. ? Endodontist. This provider treats the dental pulp of the mouth. Dental pulp is the soft interior tissue of the tooth. A common service offered by endodontists is a root canal (treatment of infected dental pulp).

? Denturist. This provider creates and delivers dentures or partial dentures to patients.

? Prosthodontist. This provider restores and replaces natural teeth. The service is offered to improve appearance and oral comfort. Common services include implants, crowns and bridges.

? Periodontist. This provider diagnoses and treats periodontitis. Periodontitis, also called gum disease, is an infection of the soft tissue surrounding your teeth.

? Oral Surgeon. This provider focuses on surgical issues of the mouth and jaws. Common services include extraction of wisdom teeth and treatment of facial injuries.

? Pediatric Dentist. This provider diagnoses, treats and manages dental health for children. They care for teeth during the stages of childhood, ranging from infancy through the teen years.

? Orthodontist. This provider diagnoses, prevents and corrects teeth, jaws and bite patterns that are out of alignment. The most common service offered by this provider is braces.

? Dental Therapist. This is a licensed oral care professional who helps your dental care team. These providers help by performing teeth cleanings and X-rays for dentists. They may even offer fillings or replace crowns.

Finding Providers in Our Network

You should get covered services from providers in our network. If you go to providers who are not in the network without first getting our OK, you may have to pay the bill. When you need urgent or emergency care and cannot get to a provider in the plan, like when you are away from home, you can go outside the plan and do not need a referral. We work with all the providers in our network to meet the needs of people with disabilities. The list of providers below shows the services they provide. If you are not sure a provider offers the services you need, talk to Member Services or your Care Coordinator.

The directory lists dentists in the network. Each area is listed alphabetically by county. Within each county, dentists are listed by specialty in alphabetical order with the address and phone number. Please visit members for the latest information.
