RELEASE OF MEDICAL RECORDS FOR CONTINUING CARE

INSTRUCTIONS

Requester will be asked to fax a written request for the records on your facility letterhead as confirmation/verification of your identity and authority to receive the records.

Note: The written request does not require patient authorization.

REQUEST MUST INCLUDE:

? Fax number to send records clearly identified ? Name of facility requesting records from ? Name of facility requesting records to be received ? Patient full name and DOB, or second identification to ensure we have identified the correct patient ? Information needed (be as specific as possible) ? Reason for request (i.e. continuing patient care)

Baker City and Ontario, at their discretion, may permit telephone requests for records when the requesting facility is known to them.

Boise and Nampa require the written request at all times. We will fax back the information requested. Please limit the request to only key information pertinent to the current visit. Twenty-five pages or less recommended. Additional information can be mailed in follow-up the next business day, if needed.
