2013-2014 STAKEHOLDER FEEDBACK DIAGNOSTIC INSTRUCTIONSThe school’s “Stakeholder Survey Report” has been created by the District based on the results of the stakeholder surveys. Below (see Section I) are the instructions to access the document. Utilize the document to complete Sections II and III of these instructions in ASSIST. However, DO NOT UPLOAD THE “STAKEHOLDER SURVEY REPORT” AT THIS TIME IN ASSIST. The demographics section of the document is still in the process of being updated. Schools will be notified when a final “Stakeholder Survey Report” will be available to upload to ASSIST. -12700021759300 Prior to completing the diagnostic in ASSIST, the school’s instructional staff members or leadership team should:Review and discuss the findings from the Stakeholder Survey Report.Review the results of the Evaluative Criteria ratings of your school.Review and discuss the following Stakeholder Feedback Diagnostic questions found in ASSIST. (It is recommended that the responses first be developed and revised in a Microsoft Word Document and then final responses copy/pasted into ASSIST.) Areas of Notable AchievementWhich area(s) indicate the overall highest level of satisfaction or approval?Which area(s) show a trend toward increasing stakeholder satisfaction or approval?Which of the above reported findings are consistent with findings from other stakeholder feedback sources? (You may compare to any source in which evidence is available. Such as 2012-2013 Climate Survey, school-based needs assessments or surveys)Areas of ImprovementWhich area(s) indicate the overall lowest level of satisfaction or approval?Which area(s) show a trend toward decreasing stakeholder satisfaction or approval?What are the implications for these stakeholder perceptions?Which of the above reported findings are consistent with findings from other stakeholder feedback sources? Such as 2012-2013 Climate Survey, school-based needs assessments or surveys) Accessing and saving your school’s Stakeholder Survey Report:Go to the OADA website using the link: Choose the Stakeholder Survey Report link.Select “Click here to create PDF file of Stakeholder Survey Report. Your document will be published on your desktop. From the “thank you screen”, go to your desktop and open the pdf file entitled “2013-2014 Stakeholder Survey Report-xxxx.pdf”, where xxxx is your 4 digit work location number. (SAVE IT ON YOUR DESKTOP ONLY - DO NOT UPLOAD TO ASSIST AT THIS TIME)Determining your Level for the Evaluative Criteria“1. Questionnaire Administration” – Every school’s rating should be a “Level 4” due to the District-wide fidelity of the administrative procedures in completing the surveys and meeting the minimum response rates. “2. Stakeholder Feedback Results and Analysis” – The school’s Level is located in your Stakeholder Survey Report under section “2. Stakeholder Feedback Results and Analysis” final results. The level was calculated based on the survey results.Answering the Stakeholder Feedback Diagnostic QuestionsUsing the Stakeholder Survey Report and the feedback from the discussions with your school’s instructional staff and/or leadership team, complete the 3 questions under “Areas of Notable Achievement” and the four questions under “Areas of Improvement”.It is recommended that the responses first be developed and revised in a Microsoft Word Document and then final responses copy/pasted into ASSIST.*REMINDERS: DO NOT UPLOAD THIS “STAKEHOLDER SURVEY REPORT” TO ASSIST.
