PLEASE READ CAREFULLY
Under the provisions of Section 1006.062, Florida Statutes, any student who is required to take medication during the
times they are attending school, including any occasion when the student is away from school property on official
school business, may be assisted by the school nurse or other designated school personnel trained in administering
medication. To ensure the student¡¯s safety, parents/guardians must comply with the following requirements:
1. A District-approved Authorization for Medication form, signed by the student¡¯s healthcare practitioner and
detailing the necessity for the medication to be provided during the school day, including any occasion when
the student is away from school property on official school business, must be submitted to the school principal.
2. A separate Authorization for Medication form must be completed for each medication to be administered during
the school day.
3. The Authorization for Medication form MUST include the method, dosage, and the scheduled time for the
medication, as well as a consent from the parent or guardian for assisting students in administrating the
prescribed medication.
4. The Authorization for Medication form needs to be filed only once during the school year. However, any changes
in the method, dosage, or schedule of the medication as directed by healthcare practitioner during the school
year will require a NEW Authorization for Medication form.
5. All medication must be received by the parent/guardian in the original container from the pharmacy or over the
counter packaging.
6. The first dose of any NEW medication MUST be given at home.
7. Any discontinued medication must be picked up from the school by the parent or guardian within one week of
the stop date. Unclaimed medication will be properly disposed of.
8. In accordance with Florida Statutes, a student may possess and use a medication to relieve headaches while
on school property or at a school-sponsored event without a physician¡¯s prescription provided the medication
is regulated by the United States Food and Drug Administration for over-the-counter use to treat headaches.
9. The parent or guardian must notify the principal if a student is in possession of medication to relieve headaches
while on school property or at a school-sponsored event.
10. The student will not share the medication or allow it to be used by any other student(s). Doing so is a violation
of the Code of Student Conduct, which may subject the student to disciplinary action.
11. The assistance in administering prescribed medication to students shall be performed by the school nurse,
school principal, or their trained designee. It is understood that there shall be no liability for civil damages
resulting from the administration of the medication when the person administering it acts as a reasonably
prudent person would under similar circumstances.

Seg¨²n las disposiciones de la secci¨®n 1006.062 de los Estatutos de Florida, cualquier estudiante que deba tomar
medicamentos durante el tiempo que asiste a la escuela, con inclusi¨®n de cualquier ocasi¨®n en la que el estudiante
se encuentre fuera del recinto escolar, por motivos oficiales en relaci¨®n con la escuela, puede ser asistido por la
enfermera de la escuela u otro personal escolar designado capacitado en la administraci¨®n de medicamentos. Para
garantizar la seguridad del estudiante, los padres de familia/tutores deben cumplir con los siguientes requisitos:
1. Se debe presentar al director de la escuela un Formulario de autorizaci¨®n para medicamentos aprobado por el
distrito, firmado por el m¨¦dico del estudiante que explique la necesidad de administrar el medicamento durante la
jornada escolar, incluyendo cualquier ocasi¨®n en la que el estudiante est¨¦ fuera del recinto escolar por asuntos
oficiales relacionados con la escuela.
2. Se debe completar un formulario de Autorizaci¨®n para medicamentos por separado para cada medicamento que
se administrar¨¢ durante la jornada escolar.
3. El formulario de Autorizaci¨®n para medicamentos DEBE incluir el m¨¦todo, la dosis y el horario programado para
el medicamento, as¨ª como el consentimiento del padre de familia o tutor para poder ayudar a administrarles el
medicamento recetado a los estudiantes.
4. El formulario de Autorizaci¨®n para medicamentos debe presentarse solo una vez durante el curso escolar. Sin
embargo, cualquier cambio en relaci¨®n con el m¨¦todo, la dosis o el horario del medicamento seg¨²n las
indicaciones del m¨¦dico durante el curso escolar requerir¨¢ un NUEVO formulario de Autorizaci¨®n para
medicamentos.
5. Todos los medicamentos deben ser recibidos por los padres de familia/tutores en el envase original de la farmacia
o en el paquete de venta libre.
6. La primera dosis de cualquier medicamento NUEVO DEBE administrarse en casa.
7. Cualquier medicamento descontinuado debe ser recogido en la escuela por el padre de familia o tutor dentro de
una semana, a partir de la fecha en la que se deja de utilizar el mismo. Los medicamentos no reclamados se
desechar¨¢n adecuadamente.
8. De acuerdo con los Estatutos de Florida, un estudiante puede poseer y utilizar un medicamento para aliviar el
dolor de cabeza mientras se encuentre en el recinto escolar o en un evento patrocinado por la escuela, sin tener
receta m¨¦dica, siempre y cuando, el medicamento est¨¦ regulado por la Administraci¨®n de Drogas y Alimentos de
los Estados Unidos, en venta libre, sin receta, con el prop¨®sito de tratar dolor de cabeza.
9. El padre de familia o tutor debe notificar al director si un estudiante lleva en su persona cualquier medicamento
para aliviar el dolor de cabeza mientras se encuentra en el recinto escolar o si asiste a un evento patrocinado por
la escuela.
10. El estudiante no compartir¨¢ el medicamento ni permitir¨¢ que ning¨²n otro estudiante lo utilice. Hacerlo es una
violaci¨®n del C¨®digo de Conducta Estudiantil, lo cual puede exponer al estudiante a medidas disciplinarias.
11. La asistencia con la administraci¨®n de medicamentos recetados a los estudiantes ser¨¢ realizada por el personal
de enfermer¨ªa en la escuela, el director de la escuela o su designado capacitado. Se entiende que no habr¨¢
responsabilidad por da?os civiles que resulten de la administraci¨®n de alg¨²n medicamento cuando quien lo
administra act¨²e como lo har¨ªa una persona razonablemente prudente en circunstancias similares.
