To be eligible for funding by the Foundation, an organization must submit, at a minimum, a

completed and signed copy of this Grant Application along with a copy of its IRS determination letter.1

Please attach supplemental page(s) if additional space is needed. All applications must be received by

the Foundation no later than September 15th to be considered for funding that year.
