Person-Centered Counseling is an interactive decision-support process in which individuals, family members and/or significant others are supported in their deliberations to determine appropriate long-term care choices, guided by the consumer’s circumstances, needs, values, and preferences. It promotes informed decisions about long-term care and supports, a primary goal of all Aging and Disability Resource Centers (ADRCs). ADRCs have a unique opportunity to meet people at times of critical decision-making and change in their lives. Person-Centered Counseling is offered when an individual has immediate or anticipated needs for long-term supports and services. It is a process, not an event, and includes multiple contacts, a documented plan, and final follow-up. How is Person-Centered Counseling different from Information/Referral? The foundation for Person-Centered Counseling is a strong Information and Referral/ Assistance (I & R/A) system. However, Person-Centered Counseling goes beyond even the most robust I&R/A programs. It builds relationships with individuals, helps them identify their goals and preferences and assists them in weighing the pros and cons of each of their various options. Counselors ensure that consumers have considered a range of possibilities when making a decision about long-term supports and they encourage planning for future needs. Person-Centered Counseling takes more time than traditional I & R/A. Person-Centered Counseling may involve in-person meetings, conversations with family members, and it requires documenting and tracking consumer outcomes over time.
