PAYMENT PLAN AGREEMENT FORM

On this day the Defendant in the above numbered and styled cause(s) agrees to the following conditions for an extended payment plan.

The Defendant acknowledges and agrees that the Court will enter a final judgment of guilt in the above numbered and styled cause(s).

The Defendant states to the Court that he/she is unable to pay the fine(s), fee(s), and cost(s) in full at this time and requests the Court to grant him/her an extended period of time to pay them in full.

The Defendant acknowledges that he/she is required to make a payment in the amount of $100.00 at the time this agreement form is executed and accepted by the Court.

The Defendant acknowledges that he/she must make a subsequent payment each and every thirty (30) days until all fine(s), fee(s), and cost(s) are paid in full.

The Defendant acknowledges that he/she understands that the extended payment plan cannot exceed one hundred twenty (120) days after the execution of this plan agreement.

The Defendant acknowledges that if said fine(s), fee(s), and cost(s) are not paid in full within thirty (30) days after the date of the execution of this payment plan that he/she is required to pay an additional twenty-five dollars ($25.00) cost in accordance with Section 133.103 of the Local Government Code.

The Defendant acknowledges and agrees that he/she is required to pay the following amount to satisfy all fine(s), fee(s), and cost(s) due to the State of Texas for the use and benefit of the City of Riesel, Texas in the above numbered and styled cause(s):

The Defendant agrees and acknowledges that in the event that he/she fails to comply with this payment plan agreement, he/she will be required to appear before the Court and show cause why the Court should not order the execution of all outstanding unpaid amounts against him/her.

The Defendant agrees and acknowledges that in the event that he/she fails to appear that a warrant will be issued for his/her arrest and that additional fees will be incurred.
