You must complete your DRIVING SAFETY COURSE and RETURN ALL DOCUMENTS to the clerk's office ON OR BEFORE 90 CALENDAR DAYS from TODAY'S PAYMENT DATE.

NO EXTENSIONS WILL BE GRANTED I understand that Judgment will be deferred for ninety (90) calendar days to allow me time to complete the Driving Safety Course approved by the Texas Education Agency and obtain a certified copy of my driving record from the Department of Public Safety.

I understand that by entering my plea, I am waiving my right to a trial by judge or jury and my right to discovery or further discovery information pertaining to my case.

I plead No Contest and agree to the following: 1. I am responsible for requesting a certified copy of my driving record from the Department of Public Safety in Austin, Texas.

a. The driving record may be ordered online at .

b. It must be a Type 3A Certified History of All Accidents and Violations in Record. (you do not need to have your driving record before you take the driving safety course)

2. I must take a Texas State Approved DRIVING SAFETY COURSE, NOT a Driver's Education Class.

? All State-approved courses are listed at tdlr.DESSearch.

I understand that the course provider has fifteen (15) days after completion of the course to furnish me with the DSC Certificate required for dismissal.

a. If attending a driving safety course to dismiss a SEATBELT VIOLATION, you MUST take a SPECIALIZED occupant & child passenger safety class.

b. If the violation occurred on a motorcycle, you MAY attend an Advanced Motorcycle Operator Training and Safety Program approved by the Department of Public Safety. State-approved motorcycle courses are listed at txdps.state.tx.us/msb.

c. I understand that the driving school will charge a fee for the course. The State court costs and administrative fine paid today are non-refundable and I will not receive a refund or credit.

3. I must return the signed `Court' copy of the Driving Safety Course completion certificate that lists `Collin County: Municipal Plano' in the `Court Information' section and the Certified Driving Record (Type 3A) to the Court for review for dismissal. If I comply with the terms of this order and my case is dismissed, the remaining fine will be waived.

Payment of the remaining fine online will constitute a waiver of the Show Cause hearing, a request for the Judge to
