Dear Parent/Guardian,
In an effort to help keep our parents and families informed regarding our commitment to creating and maintaining safe science classrooms and laboratory environments, the Denton Independent School District is asking that each parent/guardian read and discuss with their child all safety guidelines and procedures provided by their science teacher.
Each parent/guardian should be aware of the safety instructions their child will receive before participating in any laboratory activity whether in the classroom or the experimental lab room. Students will receive detailed information, both written and verbal, regarding specific safety procedures for the specific course they are currently taking.
By signing this form, you are indicating that (1) you have read all attached materials, (2) you understand that your child will be required to identify, use, and apply all laboratory procedures and guidelines, and successfully complete a safety assessment, and (3) you will instruct your child to be responsible for their behavior and to take all safety precautions for their own safety as well as the safety of their peers.
