Dear TRICARE Plus Beneficiary:

On behalf of the Commander, I am contacting you reference your enrollment in our TRICARE Plus Program. Our records indicate that you are currently enrolled in TRICARE Plus with the DeWitt Health Care Network (DeWitt ACH, Rader FHC, Fairfax FHC, or Woodbridge FHC), but are living outside of the DeWitt Health Care Network (DHCN) area. Enrollment to the DHCN requires you reside within a 30 minute drive time of one of our facilities. Your residence makes you ineligible for continued enrollment in our TRICARE PLUS program and receipt of routine primary care within the DHCN.

If our information on your residence is inaccurate, then you need to update your DEERS information by calling 1-800-538-9552, press 3, and then press 1, and contact a member of my staff No Later Than 5 February 2008. Otherwise your eligibility for TRICARE for Life benefits will remain in place but you will be disenrolled from the DHCN TRICARE Plus program and no longer assigned a primary care manager with us. You will still be eligible for emergency and pharmacy services. My staff in the Member Services Office can be reached at (xxx) xxx-xxxx.

As a TRICARE Beneficiary you are still eligible for a robust medical benefit, but you must purchase Medicare Part B to access the full spectrum of services. With Medicare Part B you are entitled to TRICARE For Life (TFL), which is a secondary payer to Medicare. To remain eligible for TFL, you must continue to pay your Medicare Part B premiums and update your military identification card. In this program Medicare will pay 80% of your medical costs, and TFL 20%. You will need to find a Medicare participating provider for you future primary care needs. You can find a Medicare participating provider by calling 1-800-633-4277 or by visiting the Medicare website at . Make sure you receive care from a Medicare participating provider and that the services you receive are covered by both Medicare and TRICARE, or you may be responsible for part or all of the cost of your care.
