Why should I enroll my child in NJIIS?
• Prevents your child from getting unnecessary shots
• Your doctors’ office can Rreminds you by mail or telephone when your child is due for shots

• Makes sure your child has all the shots needed to enroll in child care or school
• Provides a permanent record of important preventative screening tests such as newborn hearing, lead, TB, and other health screenings

Who will have access to my child’s record?
Only those persons who have been trained, signed a confidential agreement, and are authorized users can access medical and demographic information in the NJIIS.

How do I enroll my child in NJIIS?
• Children born on or after January 1, 1998 are are can be automatically enrolled in the NJIIS, , unless you signed a written declination a Declination of Newborn Automatic Enrollment form.
Ask your provider for a “Consent to Participate” form if your child was born before January 1, 1998.

How do I notify NJIIS of medical or demographic changes that need to be made to my in my child’s record?
• Some requested changes to the NJIIS record can be made by an NJIIS participating doctor or clinic. (If your child’s information is incorrect and requires changes, or if you no longer wish to participate in the NJIIS, contact your birthing facility, healthcare provider, or local/state health department.)

Can I withdraw my child from the NJIIS at a later time?
• A parent or legal guardian of a minor child may submit a
“Registrant Withdrawal From NJIIS” form which, is available at
and mail it to the address on the form.
• Any registrant 18 years of age or older may also
wWithdraw from the NJIIS in the same manner as a parent
or legal guardian.
Can I get a copy of the NJIIS record to review or keep as my copy?
• The parent or legal guardian of a minor child may submit a
“Request for Copy of the NJIIS Immunization Record” form which,
is available at and mail it to the address on the form.
• A registrant 18 years of age or older may also request a copy of the
NJIIS Immunization Record in the same manner as a parent or legal guardian.
• A copy of the NJIIS Iimmunization Record is also available from your
NJIIS participating doctor or clinic.
How will institutions with authorized access used the NJIIS?
• Child care centers, schools, and colleges use the NJIIS to assist them
iIn accessing a registrant immunization history for the purposes of
dDetermining compliance with State immunization regulations.
• Health insurance plans use the NJIIS to determine how well
their covered beneficiaries are receiving preventative heath services
and immunizations from their physicians.
• Physicians, clinics, hospitals and other health care providers use
the NJIIS to help document past immunizations and other preventative
health screening tests done elswhere. Healthcare provideers also use the
NJIIS to help insure that receommended immunizations are given on schedule.
• State and local health departments use the NJIIS to help determine
the protective level provided by vaccinations either sStatewide or on in a
specfic local community or regional basis.
