Shift Differentials: None
Work Week: Sunday – Saturday
Overtime Rate(s): 1.3 times the normal hourly rate for all hours worked in excess of 40 hours per week per state and local laws.
Holiday Rate (s): 1.4 times the normal hourly rate for All Federal Government Holidays worked
Double time Rate(s): 1.7 times for hours worked in excess of 12 hours per day.
On-Call Rate(s): On call is $5.00 per hour Vendor includes telephone consultations, when on call the individual is to carry a beeper or cell phone and answer calls. The individual must be within 30 minutes from facility or a 30 mile radius from facility or an approved radius as determined by the task ordering facility.
Call Back Rate(s): Call back is min 1 hour. If the placement is called back to the medical facility the regular rate will be charged unless overtime is incurred. Overtime is charged at the overtime rate for hours the placement worked. Advise vendor call back rates will apply once individual is physically on-site.

Charge Nurse Rate: $3.00 more than the hourly rate.

Requirements for Registered Professional Nurse
• Graduate of an accredited school of nursing – all International applicants will be in possession of VisaScreen certification as conducted by CGFNS to meet U.S. Citizenship and Immigration Services (USCIS) section 343 of the Illegal Immigration Reform and Immigrant Responsibility Act of 1996
• Current licensure in good standing in the USA
• Evidence of applicable nursing experience within the past two years
• Shall speak, understand, read and write the English language fluently. All international applicants will have taken and passed an approved English language exam to meet the U.S. Citizenship and Immigration Services (USCIS) section 343 of the Illegal Immigration Reform and Immigrant Responsibility Act of 1996
• Shall have completed all core competencies inline with Joint Commission on Accreditation of Healthcare Organization (JCAHO) quality standards for training, education, and competency
• Shall be eligible for U.S. employment and provide proof of employment eligibility
• Shall provide a statement from a physician or a report of a physical examination indicating that the individual is free from mental or physical impairments, which would restrict the individual from performing the services
• Shall provide proof of vaccination history
• Shall agree to undergo personal health examinations and such other medical and dental examinations at any time during the term of performance, as deemed necessary for preventive medicine, quality assurance or privileging purposes
• Unless vaccinated by the Government, the contractor shall be required to show proof of the vaccination. If the contractor worker declines the immunization, they must provide documentation of the waiver, which declines the influenza vaccine (declination on the basis of religious convictions or medical contraindications only).
• Individuals who have patient contact must show immunity to Measles, Mumps and Rubella (MMR) through: Serological testing which shows seroApositivity to MMR or proof of vaccination (persons born prior to 1957 must have received one dose of MMR vaccine: persons born in 1957 or later must have received two doses of MMR vaccine).
• Shall provide evidence of varicella immune status or a statement of history of chicken pox.
• On an annual basis and at own expense, contractor shall provide a current Purified Protein Derivative (PPD) reading or evaluation if known PPD reactor.

All applicants will be in possession of a credentials report from one of the USCIS approved credentialing agencies to provide documentation of equivalency of foreign-trained therapists

Requirements for Occupational Therapist
• US applicants:
Graduation from a bachelors, masters or doctoral degree program in occupational therapy accredited by the Accreditation Council for Occupational Therapy Education
• International applicants:
Graduates of foreign occupational therapy training programs also meet the education requirement if they have graduated from a degree or equivalent program recognized by the AOTA. All foreign trained occupational therapists will be in possession of credentialing completed by NBCOT (The National Board for Certification in Occupational Therapy, Inc.) The NBCOT's requirement is graduation from an occupational therapist training program that is accredited by ACOTE or approved by the World Federation of Occupational Therapists (WFOT). ACOTE accredits post baccalaureate programs, and NBCOT requires internationally-trained occupational therapists to meet the same standard.
