PHYSICIANS/SURGEONS

The probationary condition requiring Respondent to work at least 16 hours per week was amended to require Respondent to work as a physician/surgeon a minimum of 10 hours per week during the probationary period. July 2, 2012

Respondent was disciplined by the Nevada Board of Medical Examiners in February 2012. Respondent's license to practice as a physician/surgeon given a public reprimand due to disciplinary action being taken against her by the Nevada Board of Medical Examiners. Respondent's license to practice as a physician/surgeon was also placed on probation for the same period that Respondent's Nevada physician license is on probation with conditions. Respondent shall not be issued a license to administer and prescribe controlled substances in Utah until Nevada issues Respondent a controlled substance license and DEA issues Respondent a DEA registration certificate. Respondent engaged in unprofessional conduct of a sexual nature with patients under his care. Respondent's license to practice as a registered nurse is suspended until he undergoes a full psychological evaluation and a full psychosexual evaluation. Following outcome of the evaluations will determine what further action will be taken against his license. July 6, 2012 Following the completed evaluation as required in July 6, 2012 Division Order, Respondent's license to practice as a registered nurse shall remain suspended until he has successfully completed a professional boundaries course that is pre-approved by the Division. Following the period of suspension, said license will be placed on probation for five years with conditions and restrictions. July 27, 2012

Respondent pharmacy knowingly submitted false and misleading information to the

Division in order to obtain a Class B methadone pharmacy license from the Division

by failing to inform the Division that two individuals associated with the pharmacy

had felony convictions.

Order:

Respondent agreed to voluntarily surrender its licenses to practice as a Class B

pharmacy and to dispense controlled substances. Respondent's officer, Ronald

Jensen, also agreed not to reapply for licensure as a pharmacy and as a dispensary of

controlled substances for a period of five years.
