I, __________, being duly sworn depose and state the following to be true under
the penalty of perjury:
1. I am over twenty-one (21) years of age and reside at __________.
2. I am the owner and now in possession of the premises located at __________, which said
premises are more particularly bounded and described in Schedule “A” of Stewart Title Insurance Company report of Title No. __________.
3. I have continuously resided in said premises since __________.
4. I acquired said premises for good and valuable consideration from __________ by deed dated
__________, recorded __________, in the Office of the __________ of __________ County, in __________.
5. My ownership and possession has been uninterrupted and continuous; open and notorious; and
hostile and adverse to all others and exclusive of the right or claim of any person or corporation; and
title has never been questioned. No person or corporation has ever made any claim whereby my
possession or ownership might be called in question or by reason of which any claim to any part of said premises or any interest therein adverse to me might be set up.
6. The buildings and fences on said premises have been maintained in the same position and
have not been changed in any manner; no garage or other structure has been erected upon said
premises and no additions to or changes in the exterior of the buildings and fences have been made
during my ownership, except that: __________.
7. I have continuously paid the taxes and other charges in connection with said premises.
8. There are no judgments or Federal Liens against me in any Court of this State or of the United
States of America and no proceedings in bankruptcy have ever been instituted by or against me. There are no suits or proceedings pending anywhere affecting said premises and said premises, as far as I know, are free from all leases, mortgages, taxes, assessments, water charges and other liens and encumbrances, except as shown on the Title Report No. __________. No financing statements have been filed against said premises or against any fixtures or chattels attached to or used in connection with said premises; deponent is the owner of all personal property, chattels and fixtures attached to, appurtenant to or used in the operation of said premises without claim of any kind.
9. There are no facts known to me relating to the title to said premises, which have not been set
forth in this affidavit.
10. This affidavit is intended to be relied on by induce Stewart Title Insurance Company to issue a
policy of insurance covering the above stated premises.
