Please complete this form with as much information as possible. Additional requests can be addressed when a new employee has a chance to get comfortable with the MHA computer system and has had an opportunity to contact IT manager for specific, unforeseen needs.

*Remember that a desktop is ergonomically better and cheaper than a laptop. Laptop computers will only be purchased for employees that require laptops while on travel. All computers are ordered with Microsoft Office as a standard package. Keep in mind that the cost of a program increases with every program that is purchased.
