Provides direct supervision of PT and PTA and functional supervision of activities of other disciplines. Provides direct care, modeling, coaching, instruction and training on nursing care and behavioral support plans to community direct care staff and consumers following established standards and procedures of the STAR residential and Crisis Assessment Stabilization Team (CAST) mobile crisis services. SPT provides basic and general behavioral training and psychiatric nursing care to STAR residents. SPT will perform CAST services, including on-site modeling, coaching, instruction and training with consumers and their caretakers/direct care staff in in community settings, such as consumer’s homes, hospital or psychiatric settings. Must be competent in the areas of cultural sensitivity for the population being served, positive behavioral supports, trauma informed care, coaching skills training, person centered practices, and utilize less restrictive techniques to address behavioral issues. Will maintain credentials in required Senior Psychiatric Technician and required trainings. The SPT actively participates as part of the Inter-Disciplinary Team (IDT) at weekly STAR meetings, monthly IPPs, Psychotropic Drug Reviews, and mobile crisis team meetings. The SPT is a mandated reporter of observed or suspected abuse or neglect. The SPT must have at least one-year prior experience providing direct care to individuals with developmental disabilities with challenging behavior service needs and become a Registered Behavior Technician within 60 days of initial employment or be a qualified behavior modification professional. Must successfully complete competency-based training Direct Support Professional I & II, or pass the challenge test, prior to or within one year of employment. Must have a valid driver’s license and identification. SUPERVISION EXERCISED: Psychiatric Technician and Psychiatric Technician Assistant SUPERVISION RECEIVED: Unit Supervisor EXAMPLES OF DUTIES: Must possess and maintain sufficient strength, agility, endurance, and sensory ability to perform the duties contained in this duty statement. Must be able to work with consumers with developmental disabilities (e.g. autistic spectrum disorder, intellectual disability) and severe behavioral (e.g. aggression, self-injurious behavior, property destruction, and elopement) and psychiatric conditions and symptoms (e.g. psychosis, mood disorders, PTSD, and suicidal ideation). Must be able to lift 100 pounds with assistance. Must be able to complete all required training and perform and be competent in self-defense and hands-on intervention training, including two-person wall containment with resistive consumers up to 350 lbs. for up to 10 minutes; able to safely contain one limb of a combative consumer up to 5 minutes; push/pull/lift up to 50 lbs. (including laundry or medication carts); able to quickly respond to alarm within 200 feet (may occur several times daily) in interior of building; able to run 100 yards; able to assist a consumer to a standing position up to 350 lbs.; able to bend/squat up to 5 minutes. Must be able to drive up to 300 miles per day and travel to locations throughout Northern (for N. STAR) and Southern (for S. STAR) California. Travel may require over-night stays. Maintain a safe and therapeutic environment which ensures respect, dignity and protects privacy, rights, confidentiality and physical/emotional well-being of all consumers. This essential function will be ongoing when performing the following duties:10%Assists the Unit Supervisor in the organization and management of a training site and for active participation in the program’s development and direction. Includes:Direct all routine nursing activities of assigned staff and provide for total nursing care during the assigned shift. Monitors and assists staff as needed to ensure that staff knows, understand, and can carry out individuals' nursing care plans.Provide leadership and oversight to assure that staff knows, understand, and follow nursing procedures and best practices.Ensures that all staff have received proper orientation to specific training site. Provides and schedule staff training as needed to ensure staff participation in identified training.Encourages and participates in supporting facility Equal Employment Opportunity goals.Responsible for monitoring completion of appropriate documentation (incident reports, pay reporting, injury reports, medical and behavioral flow sheets, attendance records, objectives, plans and progress recordings, assessments/evaluations, time studies, etc.)Ensures licensing standards and SDC policies are followed by site staff. Responsible for site and equipment maintenance as well as security for property and personal possessions. Initiates (or reports to the Unit Supervisor) appropriate requests. Provides functional supplies and equipment at the site and maintains a neat and orderly environment. Serves as a resource to the Unit Supervisor, communicating verbally and/or in writing any concerns, problems, needs, etc. for the proper functioning of the site. Acts as Unit Supervisor during his or her absence.Maintains a safe, sanitary, and therapeutic environment that promotes and protects respect, dignity, privacy, rights, confidentiality and physical/emotional well-being of clients.10% Guide and organize assigned staff on a shift in providing active treatment/restorative care services to ensure implementation of Individual Program Plans, including:Make daily staff assignments (breaks, meals, charting, assessments, shopping, appointments, day programs, etc.) assuring full utilization of resources at all times.Assist with redistribution of staff as needed or directed to meet minimum staffing guidelines; relieve staff as needed to ensure adequate coverage.Check/monitor staffing for oncoming shift.Promote special events and actively supervise to ensure success of event.Promote clients' self- reliance in all activities and facilitate client independence.Provide leadership and guidance to assure staff facilitate and encourage clients to exercise their rights.Assist in assuring that client expenditures are appropriate and in accordance with Trust procedures.Check for safety issues and environmental needs; follow up with appropriate work orders/notifications, as needed.Check/monitor staffing for oncoming shift.Acquire and maintain knowledge of clients' backgrounds, needs, goals, and their individual program plans.Facilitate functional grouping of clients.Monitors and assists staff as needed to ensure that assigned staff on duty know, understand, and can describe individuals’ objectives and can carry out training plans.Monitor that supplies/equipment needed are available and in good repair.Give direction to staff in providing continuity of care and implementation of training.10%Communicate and collaborate with staff to carry out the facility's mission, goals, and objectives, including:Work as a team member with other Shift Supervisors to assure intershift and intra-program communication.Work as a team member at residence, program, and facility levels to provide optimal services and supports to clients and staff.Work with a multidisciplinary team to ensure that services are being provided as identified in the IPP.Coordinate the flow of communication and information via contact/exchange of information to pertinent staff on and off the residence (such as client information, announcements, restraint use, incidents, treatment issues, unusual occurrences, etc.), enter the information into the log and discuss with oncoming shift.Attend and participate in meetings as needed and assigned.Model and promote teamwork and professionalism; facilitate the shift meeting, including discussion of focus topics, changes, etc.10%Facilitate the scheduling, orientation, coaching and training of assigned employees.Conduct inter-shift review/inspection of the residence/site.Monitors and assists staff as needed to ensure that all staff is familiar with the residence routine.Monitors and assists staff as needed to ensure that staff physical and PPD appointments are kept and license renewals are completed on time.Monitor for patterns of staff tardiness or unscheduled absence, and make recommendations to the Unit Supervisor.Observe staff in all settings, orient, guide, and provide training, mentorship, and feedback regarding performance.Make recommendations to the Unit Supervisor regarding employee performance and recognition.Facilitate team building among and between shifts on the residence.10%Observes STAR mobile crisis and residence consumers’ physical condition and behavior and reports significant changes to appropriate team members.Provides behavior management support; reinforcement, intervention technique utilization including highly restrictive interventions.Collects data and records information in the record and provides follow-up as needed.Notifies residence physician/MOD and or supervisor whenever the resident has sustained injury.Initiates the incident report when required.Initiates temporary intervention plans and monitors temporary conditions.10%Performs nursing procedures, such as administering medications and treatments including oral medication, hypodermic injections, urinary catheterization, enemas, and taking and recording temperature, pulse, blood pressure, respirations, and first aid as authorized within the scope of the Psychiatric Technician nursing requirements to STAR residents.May require specialized skills in ostomy care/feeding techniques/respiratory care and adaptive equipment use and any other procedure authorized.Assist physician as necessary.Documents in treatment records medication/treatments the consumers have received prescribed medication/treatments.May assist or administer prescribed medication/treatments to consumers.Records the consumer’s response to treatment.10%Participates as a member of the Interdisciplinary Team in all aspects of the development and implementation of the STAR resident’s Habitation Plan. Escorts and STAR residents in the community. Assists staff in occupational, recreational, vocational, and educational therapy programs for consumers. Assists in the training of various Psychiatric Technician classes, such as Trainees, Psychiatric Technician Apprentices, Psychiatric Technician students.Attends all review conferences for assigned consumers. Assists with the development of crisis management goals, long range goals, skills task, behavioral plans and objectives.Is accountable for the health, safety, and welfare of the consumers they are escorting.Escorts to medical appointments, work/leisure activities, health facilities, placement visits, etc.May be required to drive a state vehicle on community outingsIncludes obtaining data and implementing objectives and plans, as well as behavior interventions.Encourage consumers to participate in planned recreational activities.Will engage consumers in activities that provide opportunities for learning and practice of skills tailored to the specific needs of the consumer.May serve as Acting Shift Lead in the absence of that person.Promotes the team concept and maintains open communication between staff, families and consumer served.10%Works at various sites as needed to meet staffing requirements. (i.e.: CPR-Behavioral Strategies Emergency response, etc.)10%Provides one-to-one supervision/direct observation of consumers on special precautions, such as elopement risks, suicide risks, serious medical condition and risk of serious self-injurious behavior to the consumers. Responds to emergencies that involve the use of medical/behavioral intervention techniques.Provides intervention that ensures safety to both consumer and staff.Monitors emergency equipment, medication, and treatment supplies.Must competently use self-defense and hands-on intervention skills to manage consumer’s assaultive behaviors.Provides documentation and follow-up as needed.5%Assists Crisis Assessment Stabilization Team (CAST) in assessment and interventions for mobile crisis referrals, including: Reviewing STAR mobile crisis referral information in collaboration with CAST; On-site (e.g. community homes, hospitals, psychiatric settings), video-conference, or telephone support assessment and support;Provides written and verbal recommendations, coaching and training to providers, Regional Center representatives, direct care staff, consumers;Documentation of services provided;Assists CAST in monitoring consumers referred to CAST.5%On-site (e.g. community homes, hospitals, psychiatric settings), video-conference, or telephone direct care support, modeling, coaching, instruction and training with consumers and their caretakers/direct care staff in community and hospital settings that includes:Assists consumers in all activities of daily living such as bathing, dressing, grooming, basic housekeeping, and dining. These duties include lifting, positioning and assisting in the consumer mobility. (Additional needs as identified by Interdisciplinary Team. (i.e.: toileting, communication, socialization, etc.).Training that will assist the consumer in maximizing their independence.Provide services which will assist each consumer in achieving maximum social, emotional, intellectual, developmental and cognitive growth.Consumer rights, ability to make choices and access shall be considered at all times.Provides positive intervention and Active Treatment for all consumers.Assists consumers in food shopping, preparation, serving and clean-up.Note: Percentages may vary based on operational needs.WORKING CONDITIONS: On-going interaction with consumers with developmental disabilities and severe psychiatric and behavioral conditions at STAR residence and in community settings, such as consumer’s homes, hospital or psychiatric settings. Potential exposure to communicable diseases, blood-borne pathogens, medicinal preparations, and other conditions common to a clinical nursing environment.
