Instructions for Reinstatement of a Lapsed License

This application must be completed and returned to the New Jersey Board of Nursing office before a clinical practice letter is issued. Along with the submission of this completed application, all fees must be paid in the form of a check or money order made out to the State of New Jersey (review Licensure Reinstatement Fee page). ................
