NYC Standardized Vulnerability Assessment

The Human Resources Administration (HRA) is introducing a new assessment to assess and prioritize individuals and heads of households that are most vulnerable for referral to supportive housing called the Standardized Vulnerability Assessment (SVA). Applicants determined eligible for any category of supportive housing will include an SVA summary.

RPMS has the following service components, each developed as a resource for people engaged in supportive housing referral work:

Referrals to supportive housing programs receiving HUD funding as well as NYC 15/15 supportive housing programs will be prioritized utilizing the SVA. For more information on the SVA please see the announcement on the next page.

In addition to the SVA, this update includes announcements from HRA regarding important changes to NYC 15/15 and the HRA 2010e application process. Please take a moment to review these announcements that include new features designed to improve and expedite the placement of individuals into supportive housing.

NYC Standardized Vulnerability Assessment

Effective January 23, 2018, all HRA 2010e applications that are approved for supportive housing will be reviewed utilizing the NYC Standardized Vulnerability Assessment (SVA). The NYC SVA will be used to readily assess and prioritize individuals and heads of households that are most vulnerable for referral to supportive housing. An individual and/or head of household's vulnerability is indicated using the following categories: High, Medium and Low Vulnerability. Applications determined eligible for any category of supportive housing will include an SVA summary outlining the factors informing the assessment.

This standardized assessment was developed in collaboration with relevant NYC and NYS government agencies, supportive housing providers, consumers and supportive housing membership organizations. The NYC SVA is in accordance with the U.S. Department of Housing and Urban Development's (HUD) issuance of 24 CFR 578.7(a)(8) and subsequent notices requiring a centralized and/or coordinated assessment system.

How does the NYC SVA impact an applicant's referral to supportive housing?

Referrals to supportive housing programs receiving HUD funding as a part of the NYC Coalition on the Continuum of Care (CCoC) will be conducted in accordance to the CCoC's written standards. To review the CCoC's written please visit their website at .

Additionally, referrals to NYC 15/15 supportive housing programs will be prioritized utilizing the SVA.

Where can I get more information on how the NYC SVA is determined?

Referring agencies completing supportive housing applications are strongly encouraged to complete the Understanding the NYC Standardized Vulnerability Assessment training prior to initiating a new application. To learn more about the factors informing the SVA review the Standardized Vulnerability Assessment Criteria Fact Sheet. Both documents can be found in the What's New section of PACT System.

Professionals to Complete Psychiatric Evaluations Expanded

Effective March 31, 2017, professionals able to complete a comprehensive psychiatric evaluation for the HRA 2010e has been expanded in order to improve access to Supportive Housing for eligible individuals and families with mental health conditions. Also, there is no longer a requirement for the supervising psychiatrist's name and license number for these professions. System enhancements have been made to allow for the selection of the clinician type who performed the evaluation. Below is the complete list of New York State licensed professionals:

? Licensed Clinical Social Worker (LCSW) ? Licensed Psychiatric Nurse Practitioner (NPP) ? Licensed Psychologist (PHD, PSYD) ? Licensed Psychiatrist (MD)

Supportive Housing Determinations for NYC 15/15

Effective September 27, 2017, all HRA 2010e applications submitted to the Placement Assessment and Client Tracking Unit will be reviewed for the NYC 15/15 supportive housing for young adult singles and young adult families. The additional categories of supportive housing available for NYC 15/15 are as follows:

Young Adult Singles

Supportive Housing for young adults (18-25 years of age) with high service utilization who are homeless or at risk of homelessness (including aging out of foster care)

Young Adult Families

Supportive Housing for young adults (18-25 years of age) with high service utilization who are pregnant or the head of household

The criteria for the NYC 15/15 supportive housing for Young Adult Singles and Young Adult Families can be found in the "What's New" section in the "Supportive Housing Description and Criteria".

NYC 15/15 is the Mayor of New York City's initiative to develop 15,000 units of supportive housing in 15 years.

HRA Implements the Coordinated Assessment Survey System

Effective September 5, 2017, HRA is offering all users of the Supportive Housing Application (2010e) the opportunity to utilize a new system called the Coordinated Assessment Survey. This system will be accessible from the same log in page and will permit movement back and forth between the two systems. The survey is a universal tool to assist case managers in understanding the different types of supportive housing and rental subsidies that are potentially available to individuals and families that are homeless or at risk of homelessness. These results may be used to inform the client's housing plan, determine the best option for placement and provide guidance for pursuing the type of housing placement that has been agreed upon. This system has been piloted with select programs since October 2016 and here are the benefits of completing the survey:

? Pilot users report the survey results for potential eligibility for the different housing types or rental subsidies is helpful in their work with individuals and families to assist in developing a permanent housing plan

? Survey returns prior supportive housing applications history and documents, including if client has active approval for supportive housing

? Applicant's identifying and income documentation that is required for placement is often available in the system

? Homeless days and benefit information is auto-populated in the survey ? Survey takes 30 minutes or less to complete for most users
