Homeless Service Provider Program - Phoenix, Arizona
Homeless Provider Program (HPP):
The Homeless Service Provider Program is for approved 501(c)3 non-profit organizations, schools or government agencies serving individuals who are “Homeless” as defined in Title 42, Chapter 119, Subchapter I, Section 11302 of the United States Code. Qualified organizations may purchase Full Local fare 1-Day, 7-Day, 15-Day and 31-Day passes at half price.
Requirements:
• Transit passes must only be issued to homeless clients for transportation to and from important activities such as: employment interviews, work, child care facilities, food/drug store, school/training and medical appointments.
• Participating agencies must track the issuance of all transit passes purchased through this program on a distribution log. The data recorded must include the distribution date, serial number of fare media, fare type, client case number (optional), client name, client signature, staff name, and reason for issue.
• Agencies must mark all transit passes issued with their City of Phoenix HPP account number.
• This program is subject to random, unannounced audits by the City or its designee.
Restrictions:
• Transit passes purchased at half price must be given only to eligible homeless clients at no charge or sold at cost.
• Passes may not be sold or given to the general public, staff members, employees, volunteers or any other client who does not meet the required criteria.
How to apply for the program:
Agencies applying for the program must provide the following via email, fax, or mail:
1) Completed HPP Fare Outlet Agreement
2) Provide a letter on your organization’s letterhead stating the organization’s:
a. Purpose/Mission
b. Description of Activities and Range of Services Provided to the Homeless
c. Geographical Service Area/s
d. Intended Use of Discounted Transit Passes
For 501c3 agencies only:
3) Provide written documentation of current 501(c)3 status
4) Provide written documentation of good standing with the Arizona Corporation Commission
