REQUIREMENTS FOR SUNY DELHI NURSING STUDENTS

FOR CLINICAL ROTATIONS

Nursing students must meet all requirements listed below. Complete the attached check list, sign the release of information form and submit all with your completed SUNY DELHI HEALTH FORM. All students participating in the clinical portion of the Nursing program must have all health records and information sent to Student Health Services, Foreman Hall, SUNY Delhi, 2 Main Street, Delhi, NY 13753 prior to July 15. Students who have not submitted all required information prior to the start of the semester will be given clinical absences that may place them in jeopardy of not meeting the objectives of the course. 1. Physical Examination - Completed SUNY Delhi HEALTH FORM within the past year and updated every two years. An Annual Health assessment is required on the alternate years. For example, if you are a freshman student who has had a physical exam within the past year prior to enrollment, only an Annual Health assessment would be required for the following year. The Annual Health assessment form is available on line and on campus. It can be completed by you while on campus and submitted to Student Health Services.

2. PPD-Mantoux - Must be less than six months old at start of classes and updated each year. If previous negative PPD is more than one year old, must have 2-step PPD (2nd given 2-3 weeks after 1st). If previous positive PPD, you must submit a copy of negative chest x-ray and record of antibiotic therapy if taken.

3. Tetanus / Td or Tdap (within 10 years) One dose of Tdap is recommended.

4. Hepatitis B Vaccine – Specify series of two or three or must have started the three dose series or have positive titer and submit a copy of the titer.

5. Two MMR’s- (measles, mumps, rubella vaccines) for all nursing students.

OR Titers – Blood tests showing immunity to all the above. Copies of Lab reports required.

6. Two Varicella vaccines (chicken pox) or Titer – (Blood test to show immunity to Chicken Pox). Copies of Lab reports required.

7. Influenza Vaccine – For freshman students given in October, available at Student Health Services.

8. Meningitis Information – Signature verifying you have received meningitis information or received vaccine (see

“part A” of the HEALTH FORM).

9. Professional Rescuer Certification in CPR – Proof of certification in CPR for the Professional Rescuer from the Red Cross or American Heart Association, must be submitted to Nursing Faculty. Certification must be in receipt by faculty or it will prevent you from participating in clinical rotations. Contact your local Red Cross, American Heart Association, EMT, or hospital in your community to obtain information of certification programs. Please contact the Nursing Department at 607-746-4492 with questions. Bring this card with you to campus…DO NOT SUBMIT TO STUDENT HEALTH SERVICES!

10. Sign the release of information on the back of the MANDATORY REQUIREMENTS check list form attached.

Copies of ALL titers and immunization record need to be attached to completed

HEALTH FORM and submitted to Student Health Services.
