Through the delivery of a service to support breast cancer patients across the Alpine Shire, that is focused on advocating and coordinating care for individuals diagnosed with breast cancer. Additionally, the position includes community based health promotion, promotion of the McGrath Foundation, group based intervention strategies and innovative strategies to meet individual needs.PRINCIPAL RESPONSIBILITIESThis position is responsible for the delivery of high quality breast care services and health promotion programs to individuals and groups.Deliver an Alpine Shire wide service to provide specialised breast care, information, education, psychological support and clinical care to clients relating to breast cancer in accordance with evidence based clinical practice guidelinesTo maintain effective and efficient patient care administrative and information systems in conjunction with Alpine Health’s policies and procedures and the Early Intervention ManagerThe ongoing health management of breast cancer patients by acting as a patient advocate and by coordinating care for people affected by breast cancer and supporting their families and carersCollaborate with members of the multidisciplinary treatment team and relevant community service providers to coordinate the delivery of client-orientated, specialised support services across the care continuum, to clients and familiesProvide leadership in community based health promotion raising ‘breast care awareness’ and promoting the McGrath Foundation throughout Alpine Health and Alpine Shire communities The promotion of the aims and objectives of the McGrath Foundation, by attending sponsorship and other promotional activities and directing interested persons to the McGrath Foundation website Participate in staff development sharing their advanced knowledge of theory and research relevant to the field of breast cancer nursing PERFORMANCE OUTCOMESClient ExperienceConduct client assessment, prioritisation, intervention, advocacy and discharge planning according to best practice guidelines and with a client-centred approach in order to achieve optimal care and outcomesMonitor the client experience and take action to improve outcomesRespond with due process to clients/carers/families’ feedback Customer service orientation, including client centred care approach Sensitive to cultural, racial and gender differencesWell-developed skills for managing challenging behaviours and demonstrating skills in conflict resolutionAble to cope with challenging behaviourRole model quality client careRespond to the analysis of changing needs of client requirementsQuality and ImprovementMaintain an up-to-date knowledge of clinical developments and research findings, incorporating best practice principles into clinical service delivery whilst maintaining professional competency standards and ethicsDemonstrate proficiency in relevant assessments, monitoring and interventionsPrioritise clinical and administrative workloads effectivelyExplore opportunities to improve services based on customer feedback and available evidenceContribute to the development of Early Intervention Team proceduresEnsure quality projects and administrative functions including statistical collection, activity monitoring and reporting are completed in an accurate and timely mannerActively participate in across-site service development projects (e.g. quality projects, cross campus education and training) as requested by the Early Intervention Manager or delegateResponsive and flexible focus on meeting operational needTeam and Relationships Collaborate effectively and respectfully with other team members and external agencies for efficient and effective patient managementMaintain an appropriate knowledge of community resources and servicesDevelop and maintain sound relationships with clients, families, carers and communityPromotes Alpine Health in the communityPresentation at in-services to the team, department and Alpine HealthWork effectively with a diverse range of people and clientsManage deliverables required for any external contractual arrangementsPOSITION REQUIREMENTSQualifications: Meets at least the minimum education requirementsDegree or higher tertiary qualification in nursing with particular relevance toBreast Cancer or oncology including; Current Australian Health Practitioner Regulation Agency (AHPRA) registration – Registered NurseRegistered nurse ideally with at least 5 years post registration experience in either surgical or medical oncology, ideally with at least three years’ experience as a breast care nurse, or related disciplineRelevant post-graduate qualifications in breast care and cancer nursing (minimum first unit of post graduate certificate, with willingness to complete the post graduate certificate in breast care nursing or cancer nursing)Desirable Previous ExperienceA proficient knowledge in the management and support of patients with breast cancer, their families and carersPrevious experience supporting people with breast cancerPrevious experience delivering group programs such as Cancer Wellness and Exercise ProgramDemonstrated ability to work within a multidisciplinary team Demonstrated ability to work as an independent practitioner within a defined support and indirect supervision frameworkProfessional ExpectationsWork with the team to provide the continuum of care to patients, provide leadership and management as appropriate and engage in continuous improvement. Demonstrated ability for effective communication, either written or verbal, with colleagues at all levels, partner agencies, clients, families and carers. Ability to develop and pursue goals in keeping with overall organisational objectives.Ability to translate standards and improvement opportunities into best practice.Current Driver’s Licence. Current National Police Check and Working With Children check Computer literate in a Microsoft environment with sound Word, Excel, Outlook and reporting skills.Demonstrated experience, knowledge and understanding of contemporary ethical issues pertinent to the practice area. Demonstrated experience and knowledge of legislation and common law pertinent to area of practice. Demonstrated understanding and commitment to Quality, Infection Prevention and Control, and WH&S principles.Demonstrated ability to apply knowledge independently and within complex situations. ................
