Acting Secretary of State Kathy Boockvar announced that 110 disciplinary actions have been taken by the Bureau of Professional and Occupational Affairs' 29 professional licensing boards, and the Division of Legislation and Notaries of the Bureau of Commissions, Elections, and Legislation:

PA Board of Accountancy

Beaver County Mercadante and Company, license no. AF046890, of Conway, Beaver County, was ordered to pay a civil penalty of $3,000 and costs of investigation of $321.49 because it held itself out as being composed of certified public accountants while its license was lapsed. (01/18/19)

PA Architects Licensure Board

Allegheny County Paul Graf, registration no. RA013660X, of Pittsburgh, Allegheny County, was ordered to pay a civil penalty of $1,000, an assessment for the costs of investigation of $265.44, and his license was publicly reprimanded because he engaged in the practice or offered to engage in the practice of architecture in the Commonwealth, or used the title of architect or used any sign, card or device implying he was competent to engage in the practice of architecture in the Commonwealth when he failed to hold a currently valid certificate issued pursuant to the Act. Out of State Stephen Phillip Maslan, PA license no. PE036362R, of Kansas City, Missouri, had his license revoked for being disciplined by the appropriate licensing authority of another state. (07/18/18)

PA Board of Auctioneer Examiners

No disciplinary actions taken.

PA Board of Barber Examiners

Philadelphia County Tealia E Torres, license no. BM093641, of Bryn Mawr, Philadelphia County, had her license to practice as a barber manager in the Commonwealth of Pennsylvania suspended for a period of one year with such suspension stayed in favor of no less than one year of probation, and was and levied a civil penalty of $250, because she was convicted of a felony. PA Board of Certified Real Estate Appraisers

Chester County Thomas Whitfield Hudlow, certificate no. GA000320L, of Phoenixville, Chester County, was ordered to pay a civil penalty of $750 and within six months, to attend and successfully complete and submit proof of the missing seven hours of education necessary to meet the requirements for the July 1, 2015-June 30, 2017, biennial reporting period because he was seven hours short of the minimum 28 total hours of continuing education required for the renewal of his certificate. Huntingdon County Jeremy Jonathan Arnold, certificate no. GA003736, of Huntingdon, Huntingdon County, was ordered to pay a civil penalty in the amount of $2,200 and an assessment fee of $800 towards costs of investigation; and was ordered to successfully complete additional hours of remedial education no later than four months after the effective date of the Order, because he violated the law by failing to complete an appraisal assignment within the requirements of the USPAP.

Montgomery County Ettore J. Petrone, license no. RL139162, of Blue Bell, Montgomery County, was ordered to pay a civil penalty in the amount of $3,000, a public reprimand was placed on his permanent disciplinary record with the Board; and he was ordered to complete the additional hours of remedial education under the terms of this agreement, because he violated the law by being suspended from the right to practice by HUD, a Federal Agency. Out of State Appraisal Logistic Solutions Inc, PA registration no. AMC000115, of Annapolis, Maryland, was ordered to pay a civil penalty of $1,250 because it violated the act by having disciplinary action taken in another state. (01/23/19)

PA Board of Chiropractic

Delaware County Patrick Trent Ryan, license no. DC006982L, of Glen Mills, Delaware County, was placed on a probation for a period of eighteen months to run concurrently with the Delaware Board of Chiropractic, retroactive to June 7, 2018, subject to the Board's terms and conditions, because he had his license suspended, revoked or refused or received other disciplinary action by the proper chiropractic licensing authority of another state, territory, possession or country. PA Board of Cosmetology

Berks County Hilary K. Yahn, license no. CO272828, of Denver, Berks County, was suspended for failing to pay a previously imposed civil penalty. (08/16/18)

Columbia County Slay Hair Studio, license no. CB127618, of Bloomsburg, Columbia County, was ordered to pay a civil penalty of $500.00 and costs of investigation of $637.20 because it acted in a grossly incompetent and unethical manner by performing microblading and it used a salon for other purposes, namely microblading. Cumberland County Special Effects For Hair, license no. CB118644, of Camp Hill, Cumberland County, was ordered to pay a civil penalty of $500 and costs of investigation of $268.85 because it acted in a grossly incompetent and unethical manner by performing microblading and it used the salon for purposes other than cosmetology, namely microblading. Monroe County Yen T. Tran, t/b/d/a Le Nails, license no. CL188727, of Brodheadsville, Monroe County, had a public reprimand placed on the permanent disciplinary record of her nail technology salon license, CY197558, and esthetician salon license, CZ119048, with the Board and these licenses to practice as a technology nail salon and esthetician salon were suspended for a period of one year, with such suspension immediately stayed in favor of probation, subject to the Board's terms and conditions; was ordered to pay a civil penalty of $5,600 and costs of investigation of $237.39 because she operated as a nail technology salon in a grossly incompetent, dishonest and unethical manner by employing an unlicensed individual, aided and abetted the unlicensed practice of cosmetology and/or nail technology by employing an unlicensed individual; possessing and use of Methyl Methacrylate substances in the practice of cosmetology; use of two metal credo tools; and failed to have the esthetician license displayed at the salon or readily available for inspection. Montgomery County Robyn G Motyl, license no. CO269019, of Hatboro, Montgomery County, was ordered to cease and desist aiding and abetting the unlicensed practice of cosmetology, was levied a civil penalty of $1,000 and an assessment for the costs of investigation of $500 because she aided and abetted the unlicensed practice of cosmetology. PA Board of Dentistry

Carbon County William F. Tinsley, license no. DS019111L, of Lehighton, Carbon County, permanently voluntarily surrendered his license to practice as a dentist in the Commonwealth of Pennsylvania, because he committed repeated acts of negligence, malpractice, or incompetence by having failed to follow the infection control recommendations issued by the Federal Centers for Disease Control in his dental practice or to ensure that auxiliary personnel and other supervisees followed these Federal guidelines. Chester County Kenneth Charles Fordham Jr, license no. DS023116L, and permit to administer nitrous oxide/oxygen analgesia, no. DN023116A, of Chester Springs, Chester County, voluntarily surrendered his license and permit, retroactive to December 21, 2018, because he engaged in unprofessional conduct. (01/24/19)

Cumberland County Robert Lloyd Myers, license no. DS030386L, of Camp Hill, Cumberland County, was ordered to pay a civil penalty of $4,000, costs of investigation of $406.73, and his license was publicly reprimanded because he made misleading or untrue representations. (01/25/19)

Out of State Craig Henry Rubinoff, PA license no. DS023335L, of Fair-Lawn, New Jersey, had a public reprimand placed on his permanent disciplinary record with the Board and paid a civil penalty of $3000, because he had a license to practice dentistry or dental hygiene or certificate for expanded function dental assisting
