Subject: Transfer of the Remains of Deceased Inmates

The State of Nevada, Department of Corrections (NDOC) is seeking proposals from qualified vendors to transfer the remains of deceased inmates at the following three facilities in Nevada. It is the NDOC's intent to enter into a contract for transferring the remains of deceased inmates and certification professionals, acting as independent contractors, who have experience in meeting the various needs of the agency. The successful vendor will provide this service, as needed or requested, for the transfer and certification for the following correctional facilities located in Nevada as listed below:

NDOC reserves the right to add or remove additional facilities during the term of the contract, upon mutual agreement between the awarded vendor and NDOC.

This contract will be administered by the NDOC and is anticipated to commence upon Board of Examiners’ approval for a term of a 4-year period (July 01, 2020 to June 30, 2024).

SCOPE OF WORK

1.1 The awarded vendor must provide the following services and certification.

1.2 NDOC is contracting with the Vendor to transfer the remains of deceased inmates housed at Ely State Prison (ESP), 4569 N SR-490, Ely, NV 89301, Ely Conservation Camp (ECC), Horse & Cattle Camp Road, Ely, NV 89301 and the Wells Conservation Camp (WCC), HC 67-50, Wells, NV 89835 to the Clark County Office of the Coroner/Medical Examiner, 1704 Pinto Lane, Las Vegas, NV 89106.

1.3 Provide a signed document from the Clark County Office of the Coroner/Medical Examiner confirming the delivery of the remains of the deceased inmate.

2. FACILITY REQUIREMENTS

2.1 Services are being conducted in active correctional facilities and should be completed during normal hours of operation (generally, between the hours of 7:00 AM and 3:30 PM; however, hours may vary by facility).

2.2 Any alternative schedules must be presented in the bid proposal.

2.3 Access is limited and requires an escort. Once the awarded vendor enters the facility, it will be difficult and time consuming to leave to get additional equipment or supplies. The awarded vendor must bring all equipment in during initial entry into the facility.

2.4 The awarded vendor should anticipate reasonable time delays due to prison operations, such as security checks (entering and exiting the facility grounds) for all contractors, subcontractors, etc. A security breach may stop or slow progress.

2.5 The awarded vendor must ensure that no equipment, etc. are left where an inmate may have access to it.

2.6 The awarded vendor must ensure that, while at the facility, inmates do not have access to telecommunication devices in accordance with NRS 209.417 and NRS 212.165.

3. SERVICE INVOICE AND/OR REPORT

3.1 A service invoice and/or report must be provided at the end of each service call and must be signed off as complete by the Facility Supervisor/designated NDOC maintenance staff.

4. PERSONNEL REQUIREMENTS

4.1 Proposals shall include the number of technicians that will be sent to each facility for service.

5. HEALTH AND SAFETY STANDARDS

5.1 The awarded vendor shall comply with all applicable federal, state, and local requirements for protecting the safety of their employees, NDOC staff, building occupants, and the environment.

5.2 All applicable standards of the Occupational Safety and Health Administration (OSHA) shall be followed when working in accordance with this Solicitation.

6. COST

6.1 Cost should be per inmate, per facility; inclusive of all labor, materials, travel/mileage, etc. necessary to perform the required services.

6.2 If the cost is not inclusive, each service provided must be specifically listed and costs must be itemized.

6.3 The attached cost sheet must be returned with proposals (Attachment CC), with pricing for each Fiscal Year (FY) of the proposed contract.

6.4 The State will not be liable for Federal, State, or Local excise taxes.

7. SERVICE AND PRODUCT GUARANTEES

7.1 Proposals should include standard, company guarantees for services provided.

8. LICENSING REQUIREMENTS

8.1 The awarded vendor must identify and provide verification of mandatory City, State and Federal licensing requirements at the time of bid submittal.

8.2 The awarded vendor must comply with all Federal, State and Local rules and regulations.

8.3 Proposals must include verification of a current Nevada State Business License (SBL).

8.4 In an effort to comply with Governor Sisolak’s Executive Order, State Purchasing is requesting ALL vendors doing business with the State to be registered in NevadaEPro. 9. TERMS AND CONDITIONS

9.1 NDOC reserves the right to reject any or all proposals received prior to contract award.

9.2 NDOC shall not be obligated to accept the lowest priced proposal, but any award made will be in the best interests of the State of Nevada after all factors have been evaluated.

10. AWARDED VENDOR EMPLOYEE BACKGROUND CHECK CLEARANCE REQUIREMENTS

10.1 Upon contract award, and at least ten (10) days prior to beginning work, the awarded vendor shall submit a completed Contractor Background Check Application (Consent for Release of Criminal History Form – DOC 50) for each employee who will be working on this project. No personnel entering the correctional facilities may be ex-felons.

11. BIDS

11.1 Bids must be received via email, no later than 2:00 PM, Tuesday, May 19, 2020 to aroman@doc.. Responses should address, at a minimum, the following:

A current State Business License;

A copy of your Certificate of Insurance;

A copy of an applicable city or county business license;

Contact person, and telephone number for service inquiries;

Company ownership and length of time in business; and

Cost.

11.2 Attached to this request you will find the State’s standard Contract for Services of Independent Contractor form for your review. Please give special consideration to Attachment BB, as this identifies the insurance limits contracted vendors will be expected to maintain for the life of the contract.

Please review the terms and conditions in this form, as this is the standard contract used by the State for all services of independent contractors. 12.2 If exceptions and/or assumptions require a change to the Contract Form, vendors must provide the specific language that is being proposed.

12.3 Please pay particular attention to the insurance requirements, which are specified in Paragraph 16 of the embedded contract and Attachment BB, Insurance Schedule.
