NOTICE OF EXAMINATION

Title:

Examination for Certificate of Fitness for Fire and Emergency Drill Conductor for NYC Pre-K to 12 Nonpublic Schools (D-20) (Premise Related)

D-20 can be obtained by qualifying for an exemption on the basis of education and experience. D-20 is only issued via Alternate Issuance Procedures (AIP) after completing training. Please see below.

QUALIFICATION REQUIREMENTS 1. Applicants must be at least 18 years of age. 2. Applicants must submit a notarized statement that self-certifies that applicant is the Principal, Assistant Principal, or otherwise serving in a role with supervisory capacity. This notarized statement must include the name of the applicant, the BEDS number of the nonpublic school, and the legal name and address of the nonpublic school. 3. Applicants must self-certify that they are physically capable to perform the critical duties, including assisting those with limited mobility, as described in the D-20 Fire and Non-Fire Emergency Drill Conductor (FEDC) for NYC Pre-K to 12 Nonpublic schools study material. 4. Applicants must self-certify that they have received a copy, in digital or print format, of the D-20 Fire and Non-Fire Emergency Drill Conductor (FEDC) for NYC Pre-K to 12 Nonpublic Schools study material. 5. Applications must include a certificate or letter provided by a Tier I or Tier II FDNY Designated Nonpublic School Agency stating that the applicant has participated in and completed General Response Protocol (GRP) training. Tier I and Tier II FDNY Designated Nonpublic School Agencies are described in Definitions Section of this document.

6. A designated liaison must submit the A-20 data electronically for multiple applicants.



application-form.pdf The data must be submitted in the layout and file type agreed upon between the Non-public School committee and the FDNY. 7. Photographs must be a recent (2x2 head shot) in JPG or JPEG format.

Special requirements for D-20 AIP: All applicants must be a Principal or Assistant Principal (a person serving in a supervisory capacity) approved by the Tier I Agency. Applicants who do not qualify MUST take the computer based FDNY administered examination and obtain an F-07/W-07 Fire and Non-Fire Emergency Drill Conductor Certificate of Fitness.

Applicants must submit a self-certifying notarized statement stating that the applicant and has received a copy of the D-20 Fire and Non-Fire Emergency Drill Conductor (FEDC) for NYC non-public Pre-K to 12 Schools study material.

APPLICATION INFORMATION Application Fees: There is a $25 application fee to FDNY to obtain a D-20 Certificates of Fitness.

RENEWAL REQUIREMENTS D-20 Certificates of Fitness are valid for three (3) years. The renewal fee is $15.The designated Tier 1 Agency coordinator shall notify the FDNY via e-mail at Elsa.Araya@fdny. of any D-20 Certificate of Fitness holders who change employment locations so that their certificate of fitness cards can be updated. Notifications should include the certificate of fitness holder's name, certificate number, and their new employment location.

You will receive a courtesy notice of renewal 90 days before the expiration date. However, it is your responsibility to renew your Certificate. It is very important to renew your C of F before it expires. Renewals submitted 90 days (up to one year) after the expiration date will incur a $25 penalty in addition to the renewal fee. Certificates expired over one year past expiration date will not be renewed. New exams will be required.

To change a mailing address: Submit a letter requesting the change of mailing address and a copy of your C of F with $5.00 fee.

To change a work location, Submit a letter from your current employer (on company letterhead) confirming that you are an employee and stating your new work location with a copy of your C of F and a $5.00 fee To request a replacement certificate: Submit a driver's license or passport, social security number, mailing address and a $5.00 fee. The certificate can be renewed On-line, by Mail or in Person.

? Renewal online If you are an individual, make sure you have your 12 digit Certificate of Fitness Access ID. This can be found on your Renewal Notice. If you do not have your Renewal Notice, your Access ID is your 8 digit Certificate of Fitness number and the last four digits of your social security number. If you are submitting renewals on behalf of a company's employees, the company must be approved by FDNY and have an 8 digit Company Code. To request approval, email pubrenew@fdny..

Renewal fee can be paid by one of the following methods: ? Credit card (American Express, Discover, MasterCard, or Visa) ? Debit card (MasterCard or Visa) ? E-check

A fee exempted applicants cannot renew online only by mail or in person.

If all the requirements are met, the certificate of fitness will be mailed out within 10 days.

? A letter requesting fee waiver on the Agency's official letterhead stating applicant full name, exam type and address of premises; AND

? Copy of identification card issued by the agency and if applicable, supporting documents to:

If all the requirements are met, the certificate of fitness will be mailed out within four to six weeks.
