This document is designed to expedite data gathering required to relocate Hewlett Packard Enterprise and multi-vendor equipment. Please complete this questionnaire thoroughly. Your HPE Project Manager will use it to develop a relocation Statement of Work and as reference tool for the project plan. All fields must be completed in order to provide a quote. It brings together the services of skilled resources in technology asset relocation and offers an insured equipment-moving service. Just as important, this service minimizes downtime and risk while reinstallation provides fast availability of essential systems at your new site.HPE Relocation Service offers flexibility, allowing you to select the type of moving services you want and need from a complete portfolio. These are available for both HPE and multivendor assets, reducing the number of vendors you have to interface with—and reducing complexity at a fixed price. Relocation Services DeliverablesHPE provides a fixed price, complete solution for customer relocations. HPE assumes full liability/responsibility for all of the equipment during the relocation.The relocation services include:HPE will assign a project manager to be the single point of contact for the customer during the entire relocation quoting, planning and delivery phases. The HPE-PM will help plan, coordinate resources and monitor relocation progress.HPE can provide services to ensure data/device security if transporting HIPAA, PII or company confidential data storage devices.HPE Relocation Services will provide multi-vendor services. The HPE-PM will engage and coordinate with the OEM to provide required technical services.An HPE-DCIS Customer Engineer (HPE-CE) will perform a site verification visit prior to the move. The HPE-CE will check travel paths, elevators and loading docks. He/she will be available to consult on the environmental requirements at the new location.On the day of the move the HPE-CE will ensure the HPE Systems are properly powered down, disconnect cables and pack any accessories so they are not damaged during transit.HPE will bring in a professional moving crew to pad, wrap and transport the equipment. Size and weight of the equipment will dictate size of the crew needed to safely transport the equipment. They will lay Masonite to protect flooring and steel plates over the gap between the floor and elevator if necessary. The HPE-CE will supervise the movers. Specialized Air-ride trucks are used for transit.Once delivered to the data center floor, the HPE-CE will re-install the equipment, re-connect power cables, power up and test the equipment. Packing materials will be provided by HPE and sealed by HPE with Tamper Evident Tape. Truck container will be sealed with numbered tag. Inventory reports must be signed by HPE and CustomerA member of the Customer workforce will be present during the entire relocation engagement, including the de-installation, packing and loading of transport phase and reinstallation of the systems. HPE will assume custody of the systems only during the transit phase. Custody of the systems during the rest of the engagement will remain with Customer. Upon delivery to the new location, representatives from HPE and Customer will verify the Truck seals, unpack and inspect.
