Discuss Making the BH Domain a project within HL7

Discussed moving project into HL7 Community Based Health Care SIG.

Discussed leveraging standards that can be adopted and adapted to support the BH data needs, e.g., the HL7 Rehabilitation Services Attachment that is required by HIPAA use for Claims Attachments may be a basis for some data.

Discussed Anasazi project to create a superset data dictionary based on a mapping of the multiple data reporting requirements. Mike anticipates the project taking from 3 – 8 months to finish this.

We discussed using the DS2000+ data standards as a base for the data superset. Sarah Minden stated that the data standards are in final edit. Kathleen and Richard will follow up.

Tom suggested that we develop a statement of this project’s mission, scope, and project plan. Kathleen suggested that the group use the HL7 template for this purpose because we have to do this anyway. Kathleen will begin filling out the template and pass it to the others to complete for discussion on the next BH Domain call. Richard asked that this work be done on a BH wiki. Kathleen will create after discussing with the other Community Based Health Care SIG cochairs.

We discussed the cultural issues for BH providers around resistance to use of non-structured data. Denny pointed out the countervailing forces in the industry that may partially explain this resistance. On the one hand, providers are encouraged to use evidence based treatments by accreditation groups and their disciplines. On the other payers and programs do not favor the use of computer based applications that standardize treatments, viewing these as cookie cutter treatment that lack individualization. We need to optimize the ways in which standards and structured data can provide flexibility to document individualized care.

Discussed the amount of effort that will be needed to develop standardized EHRs for BH, and agreed to scope the initial project to reporting requirements while making efforts to leverage data standards that are also used for EHRs purposes.

Reviewed Sacred Heart Rehabilitation Centers – Addiction Services Data Elements

The data elements listed in this early draft represent only those elements required by the addiction treatment services. CareNet and Encompass are electronic information system that Sacred Heart must use to report client demographics, service encounters and billing information to multiple regional coordinating agencies. HUD and the Genesee County Drug Court each have additional electronic information systems to which we must report. The third Party reflects insurance carriers and other payor source care managers (such as Value Options).
