? Fulfill all pre-enrollment requirements ? Register prior to the CLOSE date

Important Dates!!!

o Fall registration OPENS on May 26, 2021. o QuantiFERON TB Gold blood test must be

done 12/10/20 or later o Drug Test must be done 5/24/21 or later o Fall registration CLOSE date is at the end

of business on October 8, 2021

A complete registration file must include:

A completed Health Care Worker Criminal Background Check form (page 6 of Registration packet)

The following will already be `on record' for you if you completed them as defined in the Registration packet:

? College of DuPage application

? Advising Session attendance

? Placement test requirements

? Health requirements
