The information contained in this report summarizes new, interim, and final actions taken by the Oregon Medical Board between March 16, 2021, and April 15, 2021.

*Bailey, Douglas Dwight, MD; MD14262; Junction City, OR On April 1, 2021, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct and willful violation any Board order. With this Order, Licensee retires his medical license while under investigation.

*Carlson, Bruce Donald, MD; MD07786; Umatilla, OR On April 1, 2021, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct and willful violation any Board order. With this Order, Licensee retires his medical license while under investigation.

Chapman, Carolyn Nobuko, MD; MD27539; Portland, OR On March 29, 2021, Licensee entered into a non-disciplinary Consent Agreement for Re-Entry to Practice with the Board. In this Agreement, Licensee agreed to practice under the supervision of a pre-approved physician mentor for 500 hours, to include chart review and a report to the Board.

*Code, Patricia Anne, DPM; DP00220; Medford, OR On April 1, 2021, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct; willful violation of any Board rule; and prescribing controlled substances without a legitimate medical purpose. This Order places Licensee on probation for two years; requires Licensee to remain enrolled and in good standing in a monitoring program; and requires Licensee to maintain a therapeutic relationship with a pre-approved healthcare provider.

*Correa, Steven Day, MD; MD26987; Coos Bay, OR On April 1, 2021, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct and impairment. With this Order, Licensee surrenders his medical license while under investigation.

*Craigg, Gerald Bartholomew Roger, MD; MD22708; Walla Walla, WA On April 1, 2021, the Board issued an Order Terminating Stipulated Order. This Order terminates Licensee's January 10, 2019, Stipulated Order.

*Dew, Leigh Anne, MD; MD21140; Portland, OR On April 1, 2021, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct; impairment; and willful violation of any Board statute or rule. With this Order, Licensee retires her medical license while under investigation.

Hiebert, Eugene Lloyd, MD; MD13083; Bonney Lake, WA On March 17, 2021, Licensee entered into a non-disciplinary Consent Agreement for Re-Entry to Practice with the Board. In this Agreement, Licensee agreed to practice under the supervision of a pre-approved physician mentor for 100 anesthesia cases.

*King, Julie Ann, MD; MD23864; Medford, OR On April 1, 2021, the Board issued an Order Modifying Stipulated Order. This Order modifies Licensee's April 5, 2018, Stipulated Order by removing terms 5.1 and 5.3.

*Lang, Kathie Jean, MD; MD22164; Klamath Falls, OR On April 1, 2021, Licensee entered into a non-disciplinary Corrective Action Agreement with the Board. In this Agreement, Licensee agreed to complete pre-approved courses on management of depression and anxiety, documentation, and the diagnosis and treatment of substance use disorder.

*Metzger, Chris Allen, MD; MD28806; Minocqua, WI On April 1, 2021, the Board issued an Order Terminating Stipulated Order. This Order terminates Licensee's January 7, 2021, Stipulated Order.

*Murray, Scott Michael, MD; MD15084; Portland, OR On April 1, 2021, the Board issued an Order Terminating Stipulated Order. This Order terminates Licensee's October 8, 2015, Stipulated Order, July 13, 2017, Order Modifying Stipulated Order, and January 4, 2018, Order Modifying Stipulated Order.

Newton-Klucevek, Lisa Marie, DO; DO21207; Portland, OR On April 15, 2021, Licensee entered into a non-disciplinary Consent Agreement for Re-Entry to Practice with the Board. In this Agreement, Licensee agreed to practice under the supervision of pre-approved physician mentor for 1,000 hours.

*Paltrow, Kenneth Guy, MD; MD06784; Portland, OR On April 1, 2021, Licensee entered into a Stipulated Order with the Board for gross or repeated acts of negligence and prescribing controlled substances without a legitimate medical purpose or without following accepted procedures for examination of patients or for record keeping. With this Order, Licensee retires his medical license while under investigation.

*Ricketts, Edward Alton, Jr., MD; MD18739; Hermiston, OR On April 1, 2021, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct and prescribing controlled substances without following accepted procedures for record keeping. With this Order, Licensee retires his medical license while under investigation.

*Sandefur, Eric Todd, DO; DO20320; Baker City, OR On April 1, 2021, Licensee entered into a Stipulated Order with the Board for repeated acts of negligence. This Order requires Licensee to review the history, exam, and images for any patient aged twelve or under initially seen by a PA or other advanced practice provider for a fracture of a long bone, any fracture involving the pelvis, any intra-articular fracture proximal to the metacarpal or metatarsal bones, or any open or comminuted fracture of the humerus or femur, and requires Licensee to create or co-formulate the management plan.

*Sarver, Patrick John, MD; MD25942; Medford, OR On April 1, 2021, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct; fraud or misrepresentation in applying for or procuring a license or registration to practice in this state; and willful violation of any rule adopted by the Board. This Order reprimands Licensee; assess a $5,000 civil penalty; and places Licensee on probation for a minimum of 4 years.

*Shilland, Eric William, DO; DO27920; Portland, OR On April 1, 2021, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct; gross or repeated acts of negligence; violation of any rule adopted by the Board; and prescribing controlled substances without a legitimate medical purpose or without following accepted procedures for examination of patients or for record keeping. This Order reprimands Licensee; places Licensee on probation for a minimum of 3 years; requires Licensee to complete a pre-approved course on opiate prescribing; restricts Licensee's prescribing for chronic and acute pain; requires Licensee to taper opioid medications other than

buprenorphine/naloxone to 90 MED or less or transfer the care of the patient unless there is objective, documented evidence that such tapering is causing or likely to cause harm to the patient; prohibits Licensee from prescribing buprenorphine/naloxone to any current patient who is not already taking the medications; prohibits Licensee from initiating treatment for substance use disorder; requires Licensee to wean patients from benzodiazepines who are concurrently taking benzodiazepines with buprenorphine/naloxone and taper benzodiazepines for patients taking opioids other than buprenorphine/naloxone with benzodiazepines by 10% per month; prohibits Licensee from concurrently prescribing opioids with benzodiazepines after tapering or weaning is completed; prohibits Licensee from concurrently prescribing opioids with muscle relaxants; requires Licensee to obtain an annual EKG for any patient taking methadone; and requires Licensee to perform annual urine drugs screens and utilize the Oregon Prescription Drug Monitoring Program every three months for all patients receiving opioids.

Singh, Jeevan Kaur, LAc; AC203870; Portland, OR On April 8, 2021, Applicant entered into a non-disciplinary Consent Agreement for Re-Entry to Practice with the Board. In this Agreement, Applicant agreed to complete an 80-hour mentorship with a Board-approved clinical supervisor and complete 30 hours of NCCAOM-approved CEUs.

*Soldevilla, Francisco Xavier, MD; MD14348; Portland, OR On April 1, 2021, the Board issued an Order Terminating Stipulated Order. This Order terminates Licensee's July 10, 2020, Stipulated Order.

*Sproat, Ronald William, MD; MD12552; Prineville, OR On April 1, 2021, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct, and gross negligence in the practice of medicine. With this Order, Licensee retires his medical license while under investigation.

*Tella, Mallik Nalluri, MD; MD155385; Gresham, OR On April 1, 2021, Licensee entered into a non-disciplinary Corrective Action Agreement with the Board. In this Agreement, Licensee agreed to complete pre-approved courses on boundaries and prescribing.

*Tsen, Andrew Cho, MD; MD20029; Portland, OR On April 1, 2021, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct, and repeated negligence and gross negligence. This Order restricts Licensee from performing robotic mitral valve surgery on any patient; and requires Licensee to obtain a pre-approved practice mentor who will review indications and operative plans with Licensee prior to any mitral valve surgery and provide quarterly reports to the Board.

*Warne, Clinton Lee, MD; MD23217; Springfield, OR On March 18, 2021, Licensee entered into an Interim Stipulated Order to voluntarily withdraw from practice and place his license in Inactive status pending the completion of the Board's investigation into his ability to safely and competently practice medicine.
