PART 1: NOMINATION OF INDIVIDUAL
In accordance with: DoD Federal Acquisition Regulation Supplement Subpart 201.6, (Reference (a)), DoD Federal Acquisition Regulation Supplement, Procedures, Guidance, and Information (PGI), Subpart 201.602-2 (Reference b)), Federal Acquisition Regulation, Subparts 37.1 and 3.104 and Subpart 37.1 (Reference (c)), and the Under Secretary of Defense for Acquisition, Technology, and Logistics, Frank Kendall, Department of Defense, Instruction Number 5000.72, “DoD Standard for Contracting Officer’s Representative (COR) Certification”, 26 March 2015, the following individual is hereby nominated to perform the duties of COR:

PART II: QUALIFICATION OF NOMINEE
1) This individual has the technical and administrative abilities and the required security clearance commensurate with the proposed COR duties. The individual’s integrity and adherence to DoD 5500.7 and DoD 5500.7-R, and subpart 3.104 (Reference (b)), are above reproach. In addition, the nominee has the time available to adequately perform such duties and will be afforded all necessary resources such as equipment, supplies, and training to perform the designated functions.
2) The nominee‘s has registered in Procurement Integrated Enterprise Environment (PIEE) and completed their COR nomination within the DoD Contracting Officer’s Representative Tracking (CORT) Tool.
3) The nominee and the nominee’s supervisors understand the importance of performance of the designated functions.
4) The nominee’s performance of designated functions will be addressed as part of the individual’s performance assessments.
5) The nominee will receive notice within 30 days of appointment if an Office of Government Ethics (OGE) Form 450, Confidential Financial Disclosure Report is required.
6) Mandatory and Non-mandatory Training - The nominee has completed the following DoD Standard for Certification of COR Training Type B training. Copies of certificates are attached. Nominees who have not yet met the training requirement must successfully complete a COR training program prior to the award of the contract.
