• $10.00 fee per copy  

• $35.00 fee for a search of two to ten years if year of birth is unknown (fee includes one certification).  Additional spans of two to ten years may be searched at a rate of $25.00. 

• Please do not send cash.  Make check or money order payable to "Vital Records".

Submitting the Request

• All requests must be submitted on an application form and include the signature and valid government issued photo ID of the individual requesting the record.  Examples of acceptable identification are a state-issued driver's license or non-driver photo ID with requestor's current address, passport, etc.  If possible, photo ID should be enlarged on copier by at least 150%.

• Be certain to indicate the record is needed for genealogical research.  Applicant may provide additional information not requested on the application form on a separate sheet of paper.

Birth & Death Records Prior to 1906

Birth Records: Pennsylvania birth records prior to 1906 can be accessed through the courthouse in the county where the person was born.  The Division of Vital Records does not keep this information. 

 

Death Records: Pennsylvania death records prior to 1906 can be accessed through the courthouse in the county where the person died.  The Division of Vital Records does not keep this information.
