ATTACHMENT ASTATEMENT OF WORKRequest for Quotes (RFQ)Environmental – Waste Management – Asbestos Refresher TrainingSolicitation Number: 6100043489BACKGROUND – Asbestos Building Inspection – Initial Certification provides participants that are required to collect bulk sampling of construction materials to identify asbestos containing materials, compile inspection reports, or those who desire to understand the inspection process. Since EPA requires inspections of all buildings/structures prior to renovation and demolition, PennDOT has established inspection and reporting requirements, and some of these responsibilities fall upon Department personnel. Detailed information concerning the steps in the inspection and bulk sampling process are discussed. This class includes a 4-hour mock inspection exercise. Passing of an examination is required for individuals seeking their asbestos building inspector license.Asbestos Building Inspector – 4-hr. Refresher provides qualified candidates for recertification by PA Department of Labor and Industry (in conformance with ASHARA, 40 CFR 763) for persons who work in the asbestos abatement trades who as inspectors of buildings and other structures (most specifically bridges). This training is required for those who are licensed Asbestos Building Inspectors holding current licensure and those who have been licensed but whose license has been lapsed for less than 365 days. Those who pass the course will be eligible to apply to PA Dept. of Labor and Industry for renewal of licensure.OBJECTIVE - The objective of this project is to conduct Environmental – Waste Management – Asbestos Training courses and to develop/modify these courses as necessary on behalf of the Pennsylvania Department of Transportation (PennDOT), Bureau of Maintenance and Operations, Strategic Environmental Management Program Section.REQUIREMENTS - The term of this contract will be upon Notice of Proceed through June 30, 2019, with three (3) one (1) year options to renew by mutual consent of both parties.CONTRACTOR RESPONSIBILITIES - The awarded Contractor shall:Not use participant contact information for any reason outside the scope of this project. Not subcontract the in-classroom teaching without the expressed knowledge and consent of the TTDS Project Manager. Acknowledge that all PennDOT copyright training materials, detailed outlines of program agenda, lesson plans and facilitation aids are property of PennDOT.Have the necessary number of certified instructors available for each class scheduled. This would include a certified substitute instructor on standby for any unforeseen circumstances.For costing purposes, estimate one (1) instructor per course.Individual in the Company must be certified by the Department of Labor and Industry to instruct the course.Contractor shall obtain Department of Labor and Industry accreditation. Information on certification and other requirements can be found by visiting: Contractor shall provide a copy of the certificate for the most recent EPA or PA approved training course to the Department of Labor and Industry.Contractor shall provide copies of the approved training materials, including any PowerPoint presentations to PennDOT for duplication, along with a detailed explanation on how the materials shall be put together. PennDOT shall be responsible for all printed course materials, including any publications where applicable.Contractor shall supply and grade exams and provide certificate of completion to participants.Contractor shall have participants complete and sign the “Application for Asbestos Occupation Certification” form and forward the applications, for Commonwealth employees, to the TTDS. The Contractor shall provide a copy of the certificate of completion that must accompany the “Application for Asbestos Occupation Certification” form to be filed with the Department of Labor and Industry. Additional/substitute instructors must be approved by the TTDS Project Manager through an add employee letter and obtain accreditation from the Department of Labor and Industry.Proofread all necessary course materials for completeness, including all grammatical and punctuation errors.TASKS AND DELIVERABLESTASK A:TRAININGA1:Environmental Commitment and Mitigation Tracking System (ECMTS) Training for DesignDeliverable:One (1) course offering – Notice to proceed through June 30, 2018One (1) course offering – July 1, 2018, through June 30, 2019This is a three (3) day (21.50 hours) course intended for persons who perform environmental work, design, or rehabilitation of bridge superstructures who have never been licensed as an asbestos building inspector or for those who have held licenses, but whose license has been expired for more than one year following its expiration date. This includes Central Office/District Design and Maintenance personnel, and/or Construction personnel whose job responsibilities involve assessing and oversight of asbestos abatement activities. Upon successful completion of this course, participants shall be able to: The key aspects/elements/intent of asbestos inspections Identifying the forms of asbestos and its past uses in building andconstruction materials Health effects, exposure, and personal protective and environmentalprotection requirementsLegal liability of inspectors and required licensingPre-inspection planning, methods of inspection and selection ofrepresentative samples and minimum requirementsMethods/options of asbestos analysesA2:Asbestos Building Inspector – 4-hr. Refresher (Max 30 students)Deliverable:Two (2) course offerings – Notice to proceed through June 30, 2018Two (2) course offerings – July 1, 2018 through June 30, 2019This is a 4-hour course intended for persons who perform environmental work, design, or rehabilitation of bridge superstructures and have been licensed as an asbestos building inspector and whose licensure has not been expired for more than one year.Upon successful completion of this course, participants shall be able to: Identify and understand types of asbestos found in nature and used in industry; how it is incorporated into building materials; where it is likely to be found; how it should be safely sampled and handled for testing purposes; tools used for sampling and preservation; and chain-of-custodyApply laws and regulations regarding asbestos mitigation in building and structural inspections related to asbestos abatementUnderstand the inspector’s role in the asbestos building tradesApply for and receive licensing renewal as Asbestos Building Inspector through the PA Dept. of Labor and IndustryPerform asbestos inspections of buildings, bridges, and other structuresSample asbestos-containing material and preparing samples to be sent to an approved laboratory for analysisPresentation: Contractors shall provide a unit cost per class for Task A as shown on the Cost Matrix, Attachment B, all inclusive of the contractors cost to provide the courses as described with the exception of materials for students. The presentation cost shall include travel time to the training location, costs incurred for overnight stays, as well as subsistence entitlement. Travel costing shall conform to the requirements of the most current version of Commonwealth Management Directive 230.10, Travel Allowances which can be found at: Subsistence costing shall conform to the requirements of the most current version of U.S. General Services Administration rates and are available on the U.S. GSA website at: : PennDOT’s policy is to print course materials through PennDOT’s Graphic Services Center.? No later than forty-five (45) days prior to the first class of each fiscal year, the awarded contractor is to provide, in an electronic medium, all presentation and printable material in its original format, as well as in a PDF format, and a detailed explanation of how the materials are to be put together.Class Location: The actual location of each course will be determined by the TTDS Project Manager during the validity period of the PO resulting from this RFQ and be based on local need and a rational distribution throughout PennDOT districts. The contractor shall, for costing purposes, estimate that all trainings will be held in Harrisburg, Pa.$File/Map_of_Districts.pdf PennDOT shall provide the awarded Contractor with an instructor packet at each course location. This packet shall consist of name tents, sign-in sheets, student evaluations, an instructor evaluation, copies of the “Application for Asbestos Occupation Certification” form and a return UPS envelope. The instructor shall administer the associated course test at the end of each class offering. By electronic medium, the contractor shall submit the exam results and copies of certificates of completion. The original completed and signed “Application for Asbestos Occupation Certification” form, for Commonwealth employees, is to be mailed to Highway Admin PennDOT, 400 North Street, Harrisburg, PA, in the UPS envelope provided in the instructor packet. PennDOT will be responsible to deliver the applications and certificates of completion to the Department of Labor and Industry for processing.PennDOT will provide a standard classroom setting at each course location with a projector, screen, white board and/or flip charts, markers for both the white board and flip charts. A brief description and summary of the learning objects for each course listed in the statement of work can be found at the PennDOT Training Calendar (dot.state.pa.us/tc). The course materials are available for review during the procurement period. Please send an email to lstoneroad@ no later than 1:00 p.m. on August 2, 2017 to request a link to the course materials.TASK B: MANDATORY PLANNING SESSIONDeliverables:One (1) Mandatory Planning Session – Notice to Proceed through June 30, 2018.One (1) Mandatory Planning Session – July 1, 2018 through June 18, 2019.Within three (3) weeks of Notice to Proceed, a meeting will be scheduled by PennDOT, with the awarded Contractor, the TTDS Project Manager, and other PennDOT assigned attendees to confirm a thorough understanding of the project. The meeting shall take place at PennDOT’s central office located at 400 North Street, Harrisburg, PA, 17120 or via conference call. TTDS Project Manager. The mandatory planning session will last no more than two (2) hours. Cost shall be based on the awarded Contractor’s time of the Project Manager and one (1) instructor or developer. The Contractor shall provide a deliverable cost in the Cost Matrix, Attachment B that includes all costs associated with this task, including ics of the meeting shall include, but not be limited to: Deployment of a plan to track actual work completed against scheduled work.Attendance plans, class location plans, and a tentative class roll-out schedule.Discuss/address new issues, and communicate/resolve concerns or revised goals.Schedule of development/modifications work to be completed (if applicable).Invoicing procedures. TASK C: COURSE UPDATES, REVISIONS and/or MODIFICATIONS:Course revisions and updates may be necessary due to changes mandated by PennDOT to include, but not limited to, changes to Publications, Regulations and Initiatives. As future changes to the training material are required, the TTDS Project Manager will provide the awarded Contractor a change request outlining the course revisions. The awarded Contractor shall provide the TTDS Project Manager a proposal to include all costs and timeline revisions regarding the impact of the requested changes. All change request shall be processed in accordance with ITQ Contract Number 4400008567, Part IV – Terms and Conditions, Paragraph 21a, Changes.Upon PennDOT’s approval of the proposed changes, the Contractor will receive a purchase order or a revised purchase order giving authorization to the Contractor to proceed with the work outlined in the proposed change request. PAYMENT Payment will be based upon the timely successful completion and written acceptance of each deliverable identified in the section Tasks and Deliverables of this RFQ. Each deliverable payment will be based upon the agreed cost in the awarded contractor’s Attachment B, Cost Matrix, or as agreed to in the final Purchase Order. The PennDOT Project Manager will certify successful completion and acceptance of each deliverable and generate a “Goods Receipt” as each task is completed. PennDOT will provide the awarded vendor notice that the Goods Receipt has been generated. The awarded vendor may then send an invoice to the comptroller’s office for payment. PennDOT reserves the right to add or delete training courses identified during the period of this Purchase Order (PO). PennDOT also reserves the right to cancel classes due to low enrollment or emergencies, in addition to the reasons contained in the “Termination” provisions of the terms and conditions.
