This educational activity is jointly provided by AXIS Medical Education and Texas & New Mexico Hospice Organization

Target Audience: Executive Directors, Clinical Managers, Corporate Compliance Officers, QAPI Coordinators, the Interdisciplinary Group, and Hospice Nurses.

Overview: This is a one-day intensive for Hospice and Palliative Care leaders to engage, interact, and focus on timely issues that will lead us into the future. Throughout the intensive, participants will focus on exceptional hospice and palliative care service delivery, regulatory/policy issues, compliance concerns, dialog about how organizational leaders can more effectively engage with their staff and communities, and enhance quality of care.

| |Providing good hospice care hinges on effective documentation - from providing appropriate and |

| |effective interventions and services outlined in the plan of care to assuring full reimbursement for |

| |the entire hospice length of stay. This workshop will provide tools to document assessments completely |

| |Hospice Agencies are providing a great service to patients in their community. Yet, many agencies are |

| |struggling financially due to not being able to bill for all the days of care they are providing. Are |

| |you having non-billable days on your claims and don’t understand why? This presentation reviews the |

| |requirements related to billing that flow from your clinical practices to the claim, and provides you |

| |with the tools to review your agency practices to identify and correct those which are costing you |

| |non-billable days. |

| |management to clerical and clinical staff. This workshop will define roles in Quality Assurance |

| |Performance Improvement (QAPI) and encourage active participation in QAPI efforts. |

| |This creative and insightful presentation will investigate how cultural shifts have affected paradigms |

| |of leadership.  The Conscious Model of Leadership will allow those who attend to explore ways they can |

| |grow and develop their leadership skills.  Leaders will also be able to transform the culture of their |

| |agency and create an environment of innovation. |

| |Discuss cultural shifts and how they affect paradigms of leadership. |

| |Use the Conscious Leadership Model in order to overcome the three barriers to success. |
