Family, Domestic Relations, Consumers

Child Care Info from the Ohio Dept. of Jobs and Family Services () Information on child day care centers for parents and day care providers.

Consumer Law and Information (Cleveland Law Library Association) () Links to laws and information about consumer issues (credit, lemon laws, etc.) including statute numbers for the Ohio Revised Code.

Cuyahoga County Domestic Relations Court () Includes pull-down menus with standard, commonly-used court forms for divorce, dissolution, legal separation, child custody (parental rights and responsibilities) and child support. Includes a few post-decree motions. Some sample forms may be adapted for use in the Cuyahoga County Juvenile Court. The Domestic Relations Court docket is incorporated in the Cuyahoga County Common Pleas Court Docket. ()

Cuyahoga County Juvenile Court () Includes a very few court forms, most of them not relevant to patron needs. For sample wording of forms dealing with child custody and support issues, please try the Cuyahoga County Domestic Relations Court's Forms and Guidelines () site. The Juvenile Court Docket is included in the Cuyahoga County Common Pleas docket. () Juvenile Court handles matters involving children under the age of 18 who commit crimes or are disobedient in school or home. The Court also handles abuse, neglect, dependency, traffic, child support, and parent/child relationship cases.

Living Wills and Health Care Power of Attorney (asw/OHPCO/pt/sp/livingwills) Ohio Hospice and Palliative Care Organization and the Ohio Bar Association prepared living will forms and power of attorney forms for use by Ohio residents.

Wills, Probate, Advance Directives

Advance Directives (Living Wills, Health Care Powers of Attorney, etc.) () The Ohio Hospice and Palliative Care Organization has teamed with the Ohio Bar Association to produce a 44-page pamphlet containing instructions about how to prepare living wills, health care powers of attorney, and other advance directive documents. Users can print out individual pages of the pamphlet, such as sample wording for a living will. On , click on "Hospice and Palliative Care" in the middle of the page. Click on "Public" to open the dropdown menu, and scroll to "Living Wills" to get the "Living Wills and Advance Directives" page. The Advance Directives packet is called "Choices: Living Well at the End of Life" and can be accessed via the cover photo of the pamphlet on the right side. To get the forms in Spanish or in Large Print, scroll down the page to "Download Forms" and select the appropriate link. The pamphlets and forms all open in Adobe Acrobat Reader (as pdf documents) so we cannot provide a direct link to those items.

Cuyahoga County Probate Court () Information about the Court, probating an estate, guardianships of children and adults, living trusts, marriage licenses, and court forms. The Court website includes extensive FAQs (Frequently Asked Questions) broken down by subject (adoption, estates, guardianships and alternatives, living trusts, and marriage licenses).

Ohio Bar Association LawFacts Pamphlets (Pages/LawFactsPamphlets.aspx) This site lists pamphlets about common legal issues including wills, probate, powers of attorney, guardianships, and so on. The pamphlets can be readily printed from the website. Articles are formatted to print within the margins of letter-sized (8 1/2 x 11 inch) paper without having to search for a "printer-friendly" version. The pamphlet on "Living Wills and Health Care Powers of Attorney" (pages/lawfactspamphletsdetail.aspx?itemid=13) summarizes the information from the Ohio Hospice and Palliative Care website, but does NOT include forms.

Ohio Living Trusts (Probate Court website) () Information on how to set up and use living trusts from the Cuyahoga County Probate Court. Open the FAQs and select Living Trusts for answers to questions about living trusts, or click on the "Living Trusts" icon for a brief description.

Ohio Codes, Municipal Ordinances, U.S. Codes, Court Links

City of Cleveland Ordinances (Home/CodifiedOrdinances/tabid/66/Default.aspx) Codified Ordinances of the City of Cleveland, Ohio are available through a link on the Cleveland City Council website, or they can be accessed via a link on . ()

() The database does NOT include criminal records for Cuyahoga County Common Pleas Court, or the Common Pleas courts for Medina, Lorain, and Summit County. Court records for those counties are available on the court websites. Users should be aware that the Criminalsearches database is NOT complete, and may have data for only a few years for some courts.

Cuyahoga County Common Pleas Court () Includes information about the court, jurors, judges, etc. No motion or pleading form samples are available. The Court Docket () includes the Civil and Criminal Divisions, Domestic Relations Division, Juvenile Court Division, and Foreclosure docket. Probate Court has a separate docket. ()

Eighth District Court of Appeals (supremecourt.JudSystem/districtCourts/district8/default.asp) Handles appeals from Cuyahoga County Common Pleas Court (all divisions). The docket for the Court of Appeals is included in the Cuyahoga County Court Docket. ()

Municipal Courts in Cuyahoga County via Cleveland Law Library Association

Ohio Municipal Codes through Amlegal Publishing website (library/oh/index.shtml) Has links to municipal ordinances (codes) for Broadview Heights, Brookpark, Maple Heights, Mayfield Heights, Parma, South Euclid, and Walton Hills as well as many other cities in Ohio.

Ohio Municipal Codes through Conway Greene website (Municipal-Codes.htm) Has links to municipal ordinances (codes) for Bay Village, Bedford, Bedford Heights, Chagrin Falls, Gates Mills, Highland Heights, Independence, Lyndhurst, Mayfield Village, Orange, Strongsville, and other Ohio cities.

Ohio Supreme Court (supremecourt.) Information about the court, court opinions, announcements, etc. Includes a directory of attorneys that can be checked to see if a particular attorney is licensed and authorized to practice in Ohio, and if the attorney has been disciplined by the Court. The Court revised its website in March 2009 and expanded its options in the pull-down menu on the left frame. The Ohio Supreme Court docket is searchable back to 1985 for court cases and back to 1989 for

attorney disciplinary actions. Patrons can search by Supreme Court Case Number, party name (person or entity), prior case information (from the lower court), the name of one of the attorneys involved in the case, or other information.

Parma Municipal Court ()

Parma Municipal Court serves Parma, Parma Heights, Broadview Heights, Brooklyn, Brooklyn Heights, Linndale, North Royalton, and Seven Hills. The website does NOT have any forms, but does provide case information (docket) () under "Case Search" as well as information about representing yourself in court, the rights of defendants, the rights of victims, a complete set of the Parma Court Rules, telephone numbers for the court and court officials, links to all of the prosecutors in the communities served by the court, a Frequently Asked Questions (FAQ) section, a glossary of legal terms, and links to outside websites such as the Ohio Supreme Court and the Ohio Revised Code. U.S. Courts - Bankruptcy Courts - Northern District of Ohio

(ohnb.) There are several bankruptcy courts in the Northern Ohio area. This site links to the websites for the local courts in Cleveland, Akron, Canton, Toledo and Youngstown. The Bankruptcy Court docket is NOT available online for free. Patrons must register and login into the WebPACER system to access the docket for a small fee, or go to the Bankruptcy Court during regular business hours to use the court's public terminals for free access to the docket. Patrons may also call the Court's Voice Case Information System at 800-898-6899 using a touch-tone phone to access information about bankruptcy cases. Members of the public may do a free search on the Unclaimed Funds (ohnb.ucfquery.htm) page for funds that the Bankruptcy Trustees were unable to distribute to creditors. Library staff are not permitted to perform these tasks for patrons.

U.S. Courts - Northern District of Ohio (ohnd.) Federal district court for Ohio. The District court is the federal equivalent of a common pleas, or trial, court. The Local Rules of Court

(ohnd.Clerk_s_Office/Local_Rules/local_rules.html) for the Northern District of Ohio are available on the website. The Federal Rules of Court (rules/index.html) are available at the U.S. Courts Website. Local Courts may modify certain Federal Rules for use in that District. If there is no local court rule on a particular procedure, then the Federal Rules are used. Federal Court Dockets are NOT available free online. Patrons must register and log into the PACER system to access the Court docket. Library staff are not permitted to perform docket searches in PACER as the Library system does NOT subscribe to PACER.

U.S. Courts - Sixth Circuit Court of Appeals

(ca6.internet/index.htm) Handles appeals from federal courts in Ohio, Kentucky, Michigan, and Tennessee. Includes a limited number of court forms. Court opinions are available on the site. The Federal Rules of Appellate Procedure and Local Rules of Procedure are available under Rules and Procedures. (ca6.internet/rules_and_procedures/rulesproc.htm). The Court Docket is NOT currently available to the public for free. Patrons must register and log into the PACER system to access the Sixth Circuit docket. Library staff are not permitted to perform docket searches in PACER as the Library system does NOT subscribe to PACER.

U.S. Courts - U.S. Supreme Court ()

U.S. Supreme Court's official website, includes recent opinions and docket entries. Court opinions back to 1990 are available as .pdf documents using Adobe Acrobat Reader can be found using the volume number from the case citation. The Supreme Court docket can be searched free online but only for the current Term and the prior Term. Each Court Term runs for one year, starting on the first Monday in October.

U.S. Supreme Court cases via Findlaw website

(casecode/supreme.html) Unofficial website with U.S. Supreme Court Cases going back to 1754. Please note that free registration is required to use this site. A preferred alternative is to use the Library's subscription database, Loislaw.

( en-US/Default.aspx) Printable handbook from the County Commissioners Association of Ohio includes administrative guidelines for counties.

Cuyahoga County Home Page (cuyahogacounty.us) Main website for Cuyahoga County with links to the County Commissioners, elected officials, agencies, and courts. Some courts include usable forms or sample forms on their websites (example: the Domestic Relations Court website has sample motions and affidavits for procedures in that court).

Ohio Administrative Code (Rules and Regulations for Agencies) () Free online version of the Ohio Administrative Code (Ohio rules and regulations for government agencies and commissions) from Lawriter LLC. Search by scrolling through the Table of Contents or use the search field in the upper right corner of the screen.

Ohio Attorney General () Contains links to consumer information and complaint forms, sex offender database search, identity theft information, concealed carry laws, and other consumer-related information. Forms for various actions are also available. For more consumer information, try the Federal Trade Commission website. ()

Ohio Department of Taxation (tax.) The site has tax information and tax forms for Ohio taxpayers and businesses, including forms for previous tax years back to 1995.

Ohio General Assembly Legislative Homepage (legislature.state.oh.us) Links to Ohio bills and laws for current General Assembly as well as bills passed in prior years back to 1997 (122nd General Assembly). Also links to Ohio House and Senate homepages.

Ohio House of Representatives (house.state.oh.us) Links to contact information for Representatives searchable by name, district number, or zip code +4 number extension for your address.
