If you have been cited for one of the following violations, you are REQUIRED to appear In court on the date listed at the bottom of your citation:

Operating a vehicle while under the influence (OMVI) (PCO 333.01(a); (LCO 333.01A) ORC4511.19(A) Reckless Operation (PCO 333.02; LCO 333.02 ORC 4511.20) Physical Control (PCO 333.01(D LCO 333.01(D); ORC 4511.19(D) Leaving the scene of an accident (PCO 335.16; LCO 335.13; ORC 4549.02) Hit-Skip (PCO 335.15; LCO 335.12; ORC 4549.02-03) Driving Under Suspension (PCO 335.08; LCO 335.07 ? 335.073; ORC 4510.01 et seq) Eluding or Fleeing a Police Officer (PCO 303.01; LCO 303.01) Speeding in Excess of 20 MPH over limit, except State Speed on Freeway Drag Racing (PCO 333.07; LCO 333.07 ORC 4511.251) Child Restraint System Violation (PCO 337.29; LCO 337.26; ORC 4511.81) -2nd or more offense Any `Accident' in which citation notes an injury. Altering, Injury or Removal of Traffic Control Devices (PCO 313.08(A); LCO 313.08(A) Failure to Stop for School Bus (PCO 331.40; LCO 331.38) If you have the 3rd moving violation in a twelve (12) month period.

ALL VIOLATIONS REQUIRE PROOF OF INSURANCE FOR THE DATE OF THE CITATION. IF YOUR TICKET IS MARKED `FR NOT SHOWN' YOU MUST BRING PROOF TO COURT. DO NOT BRING INVOICES OR APPLICATIONS. YOU NEED AN INSURANCE CARD WITH EFFECTIVE DATES FOR THE CAR LISTED ON THE CITATION, OR A COPY OF YOUR INSURANCE POLICY.

FAILURE TO PROVIDE PROOF OF INSURANCE WILL RESULT IN A SUSPENSION OF YOUR LICENSE IMPOSED BY THE OHIO BUREAU OF MOTOR VEHICLES FOR A MINIMUM OF 90 DAYS.

THE FOLLOWING VIOLATIONS MAY BE WAIVED BY PAYING THE AMOUNTS SHOWN NEXT TO THE CITATION. WAIVERS MUST BE PAID IN FULL ? NO PARTIAL PAYMENTS. If you received more than one violation on the face of the traffic ticket, you must pay the amount listed for each violation.
