State Strategic Planning: Trauma System

The first goal of the FLEX EMS program is to conduct regional or state assessments and consultations. The second goal is to use the results of the consultations to create an action plan that supports strategic planning within the individual regions. Areas that have recently completed an assessment or consultation may choose to go forward with strategic planning as a Year One activity, while states that have not completed an assessment may find this activity more useful at later points during the program. States choosing to begin strategic planning during Year One should clearly document why it is an appropriate Year One activity.

States are in different places in the trauma system development process.

Suggested Activities:

1. Designate CAHs as Level III, IV, or V Trauma Facilities

2. Strategic Planning based on the Trauma-BIS Assessment or Trauma System Consultation

Overview of Activities

Designation of CAHs as Trauma Centers

Small rural hospitals are crucial to the success of the trauma system, yet almost half of the States do not designate any level III, IV or V trauma centers which represent the levels that are more typically attainable by CAH. The development and use of a statewide trauma system plan is important for CAHs because the trauma system provides guidance on comprehensive system development; allows for local trauma system variations based on assessment results (e.g. rural versus urban needs and resources); demonstrates an all-encompassing, inclusive methodology, ranging from injury prevention activities to pre-hospital trauma care, acute care facilities, and post-acute rehabilitation; and, it is increasingly required in order to be eligible for some Federal funding opportunities.

In most states, the designation of trauma centers is controlled by state law. Designation of CAHs as trauma centers (e.g. Level III, IV or V) will vary from state to state based on state criteria.

Strategic Planning

The Trauma-BIS provides state leaders with a comprehensive assessment of the status of trauma system development within the state. The Trauma-BIS provides a process for collaboration between the public health system, the EMS system and the trauma care system; a mechanism for regional and State trauma system self-assessment; the necessary structured tool to identify system gaps; a planning mechanism to promote and guide future development of State trauma care systems; and, an opportunity for improved injury care outcomes.  

Once the Trauma-BIS results are available, stakeholders should discuss development or enhancement of the state’s trauma plan. State FLEX coordinators may choose to integrate the State trauma plan with the State Rural Health Plan and may utilize the BIS assessment to establish dual priorities.

The output of an American College of Surgeons trauma system consultation is a comprehensive written assessment (upwards of 100 pages). Once the consultation results are available, stakeholders should discuss development or enhancement of the state’s trauma plan. State FLEX coordinators may choose to integrate the State trauma plan with the State Rural Health Plan and may utilize the consultation results to establish dual priorities.

Designation of CAHs as Trauma Centers

There are activities that CAHs will have to complete in order to meet the requirements of designation – for example formulating transfer policies. There will also be costs of the state agency to complete the designation process, which usually involves a team travelling to the CAH to verify the required trauma components are in place. There will likely also be equipment and training costs for nursing and physician staff.

Strategic Planning

Year One: The Trauma-BIS assessment should be followed by a facilitated strategic planning process.

Year Two and Three: Use the results of strategic planning to implement trauma improvement activities.
